美托洛尔与左卡尼汀联用对冠心病患者伴心力衰竭的临床疗效及其对心功能改善的影响

Clinical Efficacy of Metoprolol Combined with L-carnitine in Heart Failure Patients with CHD and Influence on Cardiac Function

目的:评价美托洛尔与左卡尼汀联用对冠心病患者伴心力衰竭的疗效及其对心功能和血流动力学改善的影响。方法:选取2016年1月—2018年1月间收治的冠心病伴心力衰竭患者60例资料,采用随机分组法将其为对照组和治疗组,每组30例;对照组患者给予美托洛尔治疗,治疗组患者给予美托洛尔与左卡尼汀联用治疗,比较两组患者治疗前后血流动力学和心功能指标的改善情况,以及治疗后的总有效率差异。结果:治疗后治疗组患者的总有效率(96.67%)显著高于对照组患者(80.00%)(P<0.05),心输出量、心脏指数、心搏指数显著高于对照组(P<0.05),每搏输出量显著低于对照组(P<0.05);治疗后治疗组患者LVESD、LVEDD显著低于对照组(P<0.05),LVEF显著高于对照组(P<0.05)。结论:采用美托洛尔与左卡尼汀联用治疗冠心病伴心力衰竭患者的疗效较为确切,有效改善了心功能和血流动力学参数。

Objective： To evaluat clinical efficacy ofmetoprolol combined with L-carnitine in heart failure patients with coronary heart disease （CHD） and influence on cardiac function and hemodynamics. Methods： Sixty patients with CHD who were admitted to our hospital from January 2016 to January 2018 were enrolled in the study. They were ran- domly divided into two groups. The control group was given metoprolol, The treatment group was given metoprolol combined with L-carnitine： the changes of hemodynamics and cardiac function indexes before and after treatment were compared between the two groups. Results： After treatment, the total effective rate（96.67%） in the experimental group was significantly higher than that in the control group, and the difference between the two groups was stastically signifi- cant（P〈 0.05）. After treatment, the cardiac output, cardiac index and heart rate index in the experimental group were sig- nificantly higher than those in the control group, stroke volume was significantly lower than that in the control group and the difference between the two groups was stastically significant（P〈0.05）. After treatment, LVESD, LVEDD in the experimental group were significantly lower than those in the control group, LVEF was significantly lower than that in the control group and the difference between the two groups was stastically significant（P〈0.05）. Conclusion： Metopro-lol combined with L-carnitine can effectively improve cardiac function and hemodynamics in patients with CHD.
