协同管理模式对胃癌根治术后患者并发症预防及术后康复的影响

Effect of collaborative management mode on prevention of postoperative complications and on postoperative recovery in patients with gastric cancer after radical gastrectomy

目的探讨协同管理模式对胃癌根治术后患者并发症预防及术后康复的影响。方法回顾性分析接受协同管理模式护理的59例胃癌术后患者的病历资料(观察组)和接受常规护理的56例胃癌术后患者的病历资料(对照组)。比较两组患者术后并发症的发生情况、胃肠功能的康复情况及自我护理能力。结果观察组患者并发症的总发生率为10.17%,低于对照组的25.00%,差异有统计学意义(P﹤0.05);观察组患者术后胃管拔除时间、肛门首次排气时间、下床活动时间均明显短于对照组,差异均有统计学意义(P﹤0.01);护理后,观察组患者的自我护理技能评分、自我概念评分、自我护理责任感评分、健康知识水平评分均明显高于对照组,差异均有统计学意义(P﹤0.01)。结论采用协同管理模式对胃癌根治术后患者进行护理可以有效地提高患者的自我护理能力,缩短患者术后胃肠功能的康复时间,降低术后并发症的总发生率,临床效果显著,值得推广。

Objective To explore the effect of collaborative management on the prevention of postoperative complications and on the postoperative recovery in patients with gastric cancer after radical gastrectomy. Method A retrospective analysis on the medical data of 59 gastric cancer patients who were administered with collaborative management for postoperative nursing care（study group） and another 56 cases that received conventional nursing care（control group） was performed. The incidence of postoperative complications, recovery of gastrointestinal function and self-care abilities were compared between the two groups. Result The incidence of complications in study group was 10.17%, and was lower than that in the control group at 25.00%, and there was significant difference between the two groups（P〈0.05）; the time to removal of nasogastric tubes, time to first anal exhaust, and time to ambulation in study group were shorter compared with those in control group, indicating statistically significant difference（P〈0.01）; after nursing care, the self-care skills score, self-concept score, sense score of self-care responsibility and health knowledge score were all better in study group than in control group, and the difference was of statistical significance（P〈0.01）. Conclusion The use of collaborative management for nursing care in patients receiving radical gastrectomy can effectively improve the self-care skills, shorten the time needed for gastrointestinal recovery after surgery, and may reduce the overall incidence of postoperative complications, showing significant clinical effect and clinical feasibility.
