术中使用气压波动对术后静脉血栓栓塞的预防效果

PreventiveEffect of Air Wave Pressure in Operation on Venous Thrombus Embolism after Operation

目的评价气压波动预防消化道肿瘤患者术后深静脉血栓和肺栓塞的临床效果。方法将需要手术治疗的胃癌和结直肠癌患者随机分为治疗组和对照组,治疗组53例,无干预组49例,治疗组在术中给予气压波动治疗,无干预组不给予预防治疗。术前常规进行下肢深静脉超声检查,术后第6天检测肺栓塞、深静脉血栓发生和腘静脉流速,比较两组之间有无差异。结果两组患者的性别、年龄和病种分布无显著性差异。治疗组和对照组术后第6天的平均腘静脉流速分别为(48. 2±7. 6)cm/s和(46. 8±6. 7)cm/s(P=0. 736)。治疗组和无干预组的深静脉血栓发生率分别为5. 7%和22. 4%,两组对比差异有统计学意义(P=0. 02)。无干预组术后发生肺栓塞一例,治疗组无肺栓塞发生(P=0. 48)。结论术中气压波动治疗可有效预防消化道肿瘤患者术后静脉血栓栓塞的发生。

Objective To discuss the clinical effect of air wave pressure in preventing the occurrence of venous thrombus embolism （VTE） after operation in cases with gastrointestinal tumors. Methods 102 cases with gastric or colorectal cancer were randomly divided into 2 groups ： treatment group （ n = 53 ） and non - intervention group （ n = 49 ） ; air wave pressure was given to cases in treatment group in operation; uhrasound examination of lower limb deep veins was routinely made to cases in both groups before operation and on the 6 th day after operation, the occurrence of pulmonary embolism and deep venous thrombosis, as well as the velocity of popliteal vein, were detected, a comparative study was made between the 2 groups. Results The basic lines of the cases in the 2 groups were of no obvious difference ; on the 6 th day after operation, the average velocity of popliteal vein was 48.2 ± 7.6 cm/s in treatment group and 46. 8 ±6.7cm/s in non -intmw-ention group （P =0. 736） ; the occurrence of deep venous thrombosis in the 2 groups was 5.7% and 22.4% respectively, the difference was of statistical significance （P = 0.02 ） ; pulmonary embolism occurred in no case in treatment group while in 1 case in non -intmw-ention group （P = 0.48 ）. Conclusions Air wave pressure in operation can effectively prevent the occurrence of VTE after operation in cases with gastrointestinal tumors.
