镇静安神汤联合米氮平治疗焦虑抑郁状态伴难治性失眠患者的临床效果观察

Clinical effects of Zhenjing Anshen decoction combined with Mirtazapine in treatment of patients with anxiety and depression complicated with refractory insomnia

目的:观察镇静安神汤联合米氮平治疗焦虑抑郁状态伴难治性失眠患者的临床效果。方法:选取60例焦虑抑郁状态伴难治性失眠患者作为研究对象,依据随机数字表法将其分为观察组和对照组各30例。对照组采用米氮平治疗,观察组在对照组基础上联合镇静安神汤治疗。比较治疗前后两组汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)及匹兹堡睡眠质量指数(PSQI)等评分、临床效果及不良反应发生情况。结果:治疗后观察组HAMD、HAMA及PSQI评分均明显低于对照组,差异有统计学意义(P<0.05);观察组治疗有效率为93.33%(28/30),明显高于对照组的76.67%(23/30),差异有统计学意义(P<0.05);观察组不良反应率为6.67%(2/30),明显低于对照组的20.0%(6/30),差异有统计学意义(P<0.05)。结论:镇静安神汤联合米氮平治疗焦虑抑郁状态伴难治性失眠患者的临床效果优于单纯米氮平治疗效果。

Objective： To explore clinical effects of Zhenjing Anshen decoction combined with Mirtazapine in treatment of patients with anxiety and depression complicated with refractory insomnia. Methods： 60 patients with anxiety and depression complicated with refractory insomnia were selected as the research objects, and randomly divided into two groups, 30 cases in each group. The control group （n=30） was treated with Mirtazapine, while the observation group was treated with Zhenjing Anshen decoction combined with Mirtazapine. The scores of Hamilton depression scale （HAMD） and Hamilton anxiety scale （HAMA） as well as Pittsburgh sleep quality index （PSQI） before and after the treatment, the clinical effects and adverse reactions were compared between the two groups. Results： After the treatment, the HAMD, HAMA and PSQI scores in the observation group were signifcantly lower than those in the control group, and the differences were statistically signifcant （P〈0.05）. The effective rate in the observation group was 93.33% （28/30）, which was signifcantly higher than 76.67% （23/30） in the control group, and the difference was statistically signifcant （P〈0.05）. The adverse reaction rate of the observation group was 6.67% （2/30）, which was signifcantly lower than that of the control group （20.0%,6/30）, and the difference was statistically signifcant （P〈0.05）. Conclusions： Zhenjing Anshen decoction combined with Mirtazapine in the treatment of the patients with anxiety and depression complicated with refractory insomnia is superior to single Mirtazapine.
