开腹手术和腹腔镜治疗结石性胆囊炎的疗效

Efficacy of open cholecystectomy and laparoscopic cholecystectomy for calculous cholecystitis

目的观察和对比开腹胆囊切除术和腹腔镜胆囊切除术治疗结石性胆囊炎的临床效果。方法选择我院收治的82例结石性胆囊炎患者作为研究对象,将其随机分成对照组和研究组,每组41例。对照组行开腹胆囊切除术治疗,研究组行腹腔镜胆囊切除术治疗。对比两组疗效。结果研究组患者的手术时间、肛门首次排气时间及住院时间均明显短于对照组,手术治疗费用明显高于对照组,术后VAS评分、血清CRP水平及术后并发症总发生率均明显低于对照组(P<0.05)。结论腹腔镜胆囊切除术治疗结石性胆囊炎具有手术时间短、术后恢复快、并发症少等优势,而开腹胆囊切除术的手术费用相对较低,因此患者可依据自身情况合理选择不同的术式进行治疗。

Objective To observe and compare the clinical effects of open cholecystectomy and laparoscopiccholecystectomy in the treatment of calculous cholecystitis. Methods Eighty-two patients with calculous cholecystitis whoadmitted to our hospital were selected as study objects and randomly divided into control group and study group, with 41cases in each group. The control group underwent open cholecystectomy. The study group was treated with laparoscopiccholecystectomy. The efficacy of the two groups were compared. Results The operation time, first anal exhaust time andhospitalization time of the study group were significantly shorter than those in the control group, the cost of surgery wassignificantly higher than that of the control group, postoperative VAS score, the level of serum CRP and the total incidence ofpostoperative complications in the study group were significantly lower than those in the control group （P〈0.05）. ConclusionThe laparoscopic cholecystectomy in the treatment of calculous cholecystitis has the advantages of short operation time, rapidpostoperative recovery and few complications. However, the open cholecystectomy has relatively low cost of surgery.Therefore, patients can choose different surgical methods according to their own conditions.
