不同术式对高位复杂肛瘘手术患者近远期疗效的影响

Effect of different surgical methods on the short-term and long-term effects in patients with high complex anal fistula surgery

目的研究不同术式对高位复杂肛瘘手术患者近、远期疗效的影响。方法选取2016年2月至2017年2月于我院进行治疗的高位复杂肛瘘患者92例,随机分为对照组与观察组,每组46例。对照组患者行传统切开挂线术,在此基础上,观察组患者行切开挂线对口引流术。比较两组患者的治疗效果及排便功能相关的生活质量。结果观察组患者的治疗总有效率为97.83%,明显高于对照组的80.43%(P=0.01)。观察组术后1年的生活方式、心理应付/行为评分均明显高于对照组,抑郁/自我感受、窘迫尴尬评分均明显低于对照组(P<0.05)。结论与传统切开挂线术相比较而言,切开挂线对口引流术治疗高位复杂肛瘘的近、远期疗效更为理想,值得推广实践。

Objective To study the different surgical methods on the short-term and long-term effects in patients with highcomplex anal fistula surgery. Methods A total of 92 patients with high complex anal fistula treated in our hospital from February2016 to February 2017 were selected. All patients were randomly divided into control group and observation group, with 46cases in each group. The control group was treated with traditional incision and thread-drawing, on this basis, the observationgroup was treated with incision and thread-drawing drainage. The treatment effects and the quality of life associated withdefecation function in patients of the two groups were compared. Results The total effective rate of treatment in the observationgroup was 97.83%, which was significantly higher than 80.43%of the control group（P=0.01）. At 1 year after surgery, the scores oflifestyle and psychological coping/behavior in the observation group were significantly higher than those of the control group,and the scores of depression/self-feeling and distress were significantly lower than those of the control group（P〈0.05）. ConclusionCompared with traditional incision and thread-drawing, the incision and thread-drawing drainage in the treatment of highcomplex anal fistula has more ideal short-term and long-term effects, it is worth popularization and practice.
