摘要
目的探讨汉族、维吾尔族女性不同分子分型乳腺癌的MRI表现。方法回顾性搜集具有术前3.0T MRI检查及术后病理的222例乳腺癌患者资料,分为汉族(n=122)、维吾尔族(n=100)两组,观察腺体背景、背景强化程度、肿瘤形态、边界、边缘毛刺、强化方式、时间-信号强度曲线类型及达峰时间;测量肿瘤最大径、ADC值、初始T_1值、强化峰值并计算最大强化率;根据术后病理结果,采用χ~2检验比较汉族、维吾尔族各分子亚型(Luminal A型、Luminal B型、HER-2过表达型、三阴性)间MRI表现的差异;并对ADC值、初始T_1值、强化峰值、最大强化率进行方差分析和t检验,P〈0.05为差异具有统计学意义。结果汉族与维吾尔族不同分子分型乳腺癌肿瘤边缘有无毛刺的差异具有统计学意义(P〈0.05);汉族不同分子分型乳腺癌之间ADC值的差异具有统计学意义(P〈0.05);维吾尔族不同分子分型乳腺癌之间肿瘤的初始T_1值差异具有统计学意义(P〈0.05);汉族与维吾尔族HER-2过表达型乳腺癌初始T_1值之间差异具有统计学意义(P〈0.05);汉族与维吾尔族Luminal A型乳腺癌之间最大强化率差异具有统计学意义(P〈0.05)。其余差异均无统计学意义(P〉0.05)。结论汉族、维吾尔族不同分子分型乳腺癌MRI表现有一定差异,MRI或可成为预测乳腺癌分子亚型的无创性手段,为不同民族的个体化治疗提供帮助。
Objective To investigate the MRI findings of different molecular types of breast cancer in Han and Uygur women.Methods A retrospective collection of 222 cases of breast cancer patients with preoperative 3.0 T MRI and postoperative pathology was divided into Han(n=122)and Uygur(n=100).Two groups were observed for gland background,background enhancement tumor degree,morphology,boundary,edge burr,enhancement mode,time signal curve type and peak time;measured tumor maximum diameter,ADC value,initial T1 value,enhanced peak value and maximum enhancement rate;According to the postoperative pathological results,theχ~2test was used to compare the MRI findings of the subtypes of Han and Uygur(Luminal A,Luminal B,HER-2 overexpressed,triple negative);ADC values,initial T_1Value,enhancement peak,maximum enhancement rate were analyzed by ANOVA and T test.P〈0.05 was considered statistically significant.Results There were significant differences in the presence or absence of burrs between the Han and Uighur different molecular types of breast cancer(P〈0.05).The difference of ADC values between different molecular types of breast cancer in Han nationality was statistically significant(P〈0.05).There was a statistically significant difference in the initial T_1values between Uygur patients with different molecular types of breast cancer(P〈0.05),Han nationality There was a statistically significant difference between the initial T_1values of Uygur HER-2 overexpressing breast cancer(P〈0.05).The maximum enhancement rate between Han and Uygur Luminal A breast cancer was statistically significant(P〈0.05).).The rest were not statistically significant(P〉0.05).Conclusion MRI manifestations of different molecular types of breast cancer in Han and Uygur patients are different.MRI may be a non-invasive means for predicting molecular subtypes of breast cancer,and it can help individualized treatment of different ethnic groups.
