血清半乳甘露聚糖抗原水平在真菌性鼻窦炎患者诊断中的应用价值

摘要 目的探讨血清半乳甘露聚糖抗原水平在真菌性鼻窦炎患者诊断中的应用价值。方法选取2016年1月至2017年1月本院耳鼻喉科收治的住院患者134例。根据患者的病理学检查结果不同,将入选患者分为对照组(非真菌性鼻窦炎)和观察组(真菌性鼻窦炎),其中对照组72例,观察组62例。比较两组患者的血清半乳甘露聚糖抗原水平。观察GM试验诊断真菌性鼻窦炎的敏感度、特异度、阴性预测值以及阳性预测值。结果观察组阳性率高于对照组,阴性率低于对照组,差异具有统计学意义(P<0.05);GM试验的检测敏感度为0.29,特异度为0.91,阴性预测值为0.59,阳性预测值为0.74,诊断符合度为0.64。结论 GM试验可作为真菌性鼻窦炎的早期诊断方法,具备较高的特异度,值得进一步推广应用。 Objective To investigate the diagnostic value of serum galactomannan antigen level in patients with fungal sinusitis. Methods: 134 cases of hospitalized patients admitted to Ear-Nose-Throat Department in our hospital from January 2016 to January 2017 were selected. According to the pathological examination results of the patients, the patients were divided into control group (non fungal sinusitis) and observation group (fungal sinusitis), of which 72 cases were in the control group and 62 in the observation group. Serum galactomannan antigen levels were compared between the two groups. The sensitivity, specificity, negative predictive value and positive predictive value of GM test in the diagnosis of fungal sinusitis were observed. Results: The positive rate of the observation group was higher than that of the control group, and the negative rate was lower than that of the control group. The difference was statistically significant (P〈0.05). The sensitivity of the GM test was 0.29, specificity was 0.91, negative predictive value was 0.59, positive predictive value was 0.74, and diagnostic coincidence rate was 0.64. Conclusion: GM test can be used as an early diagnostic method for fungal sinusitis. It has high specificity and is worthy of further promotion and application.
