超声引导穿刺硬化剂换洗治疗乳腺囊肿的研究

Curative Effect of Ultrasound-guided Puncture on Patients with Breast Cyst

目的探讨乳腺囊肿患者进行超声引导穿刺治疗的临床效果。方法选择华南农业大学医院2017年1月1日—2018年5月31日收治的67例乳腺囊肿患者进行超声引导穿刺抽液硬化剂换洗治疗,其中32例采用超声引导下穿刺抽液用高渗糖水换洗硬化治疗作为A组; 35例采用无水乙醇换洗硬化治疗作为B组;收集2017年前行外科手术切除的乳腺囊肿患者31例作为C组,将3组病例治疗效果及不良反应进行分析比较。结果 ABC三组患者的总有效率分别为93. 33%、95. 83%、77. 50%,AB两组患者总有效率明显高于C组,P <0. 05; AB两组组间比较没有统计学差异,P> 0. 05; A组患者不良反应的发生率6. 25%,明显低于B组患者28. 57%,P <0. 05。结论在超声引导下实施乳腺囊肿穿刺抽液,采用换洗硬化剂高渗糖水或无水乙醇换洗治疗,对患者伤害较少,疗效较传统手术强,而高渗糖水的应用范围更大,值得推广应用。

Objective To explore the clinical effect of ultrasound-guided puncture on patients with breast cyst. Methods Sixty-seven patients with breast cyst were admitted to South China Agricultural University Hospital from January 1, 2017 to May 31, 2018. They were treated with ultrasound-guided puncture and sclerosing agent exchange. In group A, 32 cases were treated with ultrasound- guided puncture and drainage with hypertonic syrup,in group B, another 35 cases were treated with anhydrous ethanol for sclerotherapy and group C was composed of 31 cases of breast cysts that underwent surgical resection before 2017. The he three groups were compared in terms of curative effects and adverse reactions. Results The overall response rates of group A, B, and C were respectively 93.33 %, 95.83 %, 77.50 %, and the overall response rates of groups A and B were significantly higher than that of group C, P 〈0.05; there was no significant differences between group A and B （ P 〉0.05）. The incidence of adverse reactions in group A was 6.25 %, which was significantly lower than that of group B 28.57 % （ P 〈0.05）. Conclusion Under the guidance of ultrasound, the puncture and drainage of breast cyst is carried out, and the treatment is carried out by changing the sclerosing agent with high osmotic sugar or anhydrous alcohol. They render less harms to the patients, and the curative effect of them are better than that of traditional operations, so they are worthy of popularization.
