注射垂体后叶素在异位妊娠腹腔镜下切开取胚中的作用

Effect of pituitrin injected on ectopic pregnancy by laparotomy

目的探讨在异位妊娠腹腔镜下切开取胚中注射垂体后叶素的效果。方法选取2015年4月～2018年6月在我院行腹腔镜下切开取胚术的异位妊娠患者86例作为研究对象,采用随机数字表法将其分为对照组(术中不用垂体后叶素治疗)和观察组(术中注射垂体后叶素治疗),各43例。对比两组患者的术中止血时间、出血量、手术时间以及术后情况。结果观察组的止血时间(4.23±1.03)min、术中出血量(49.08±5.70)mL手术时间(27.02±4.22)min、血β-HCG转阴时间(16.14±1.97)d以及持续性异位妊娠发生率(0.00%)均显著低于对照组的止血时间(13.01±1.20)min、术中出血量(75.40±5.40)mL手术时间(67.25±4.11)min、血β-HCG转阴时间(22.05±2.03)d以及持续性异位妊娠发生率(11.63%);观察组的输卵管通畅率为90.70%显著高于对照组的76.74%,差异有统计学意义(P<0.05)。结论在异位妊娠腹腔镜下切开取胚中,注射垂体后叶素治疗效果显著,能够有效缩短止血时间、血β-HCG转阴时间,降低持续性异位妊娠发生率,提高输卵管通畅率。

Objective To investigate the effect of pituitrin injection into the embryo of ectopicpregnancy by laparoscopic incision.Methods 86 patients with ectopic pregnancy who underwent laparoscopicembryo extraction in our hospital from April 2015 to June 2018 were divided into control group （withoutintraoperative pituitrin treatment） and observation group （intraoperative injection of pituitrin treatment）,43 cases in each group. The hemostatic time, bleeding volume, operative time and postoperative conditionwere compared between the two groups.Results The hemostasis time （4.23 ＋1.03） min, the bleedingvolume （49.08±5.70） ml operation time （27.02 ＋4.22） min, the blood beta-HCG negative conversion time（16.14±1.97） D and the incidence of persistent ectopic pregnancy （0.00%） in the observation group weresignificantly lower than those in the control group （13.01±1.20） min, the bleeding volume （75.40±5.20）min. 40） ml operation time （67.25±4.11） min, blood beta-HCG negative time （22.05±2.03） D and persistentectopic pregnancy rate （11.63%）. The tubal patency rate of observation group （90.70%） was significantly higherthan that of control group （76.74%）.The difference was statistically significant （P〈0.05）.Conclusion Inlaparoscopic embryo extraction of ectopic pregnancy, pituitrin injection has a significant therapeutic effect,can effectively shorten the hemostasis time,blood beta-HCG negative time,reduce the incidence of persistentectopic pregnancy,improve the tubal patency rate.
