摘要
目的探讨经阴道二维超声联合经阴道三维超声诊断剖宫产术后切口妊娠的临床价值。方法回顾性分析79例手术诊断为剖宫产术后切口妊娠病例,进行二维及三维超声图像诊断,比较剖宫产术后切口妊娠的符合率及不同切口处肌层厚度超声诊断的符合率。结果二维联合三维超声诊断符合率(96.20%)较二维超声诊断符合率(87.34%)高,两者差异有统计学意义(P<0.05);在子宫前壁下段切口处肌层>2 mm时,三维联合二维超声诊断符合率(97.56%)较二维超声诊断符合率(80.49%)高,两者差异有统计学意义(P<0.05);在子宫前壁下端切口处肌层<2mm时,二维联合三维超声诊断符合率与二维超声诊断符合率相同。结论二维联合三维超声较二维超声诊断价值高,尤其在子宫前壁下段切口处肌层厚度>2mm时更具有优势。
Objective To investigate the clinical value of combined two-dimensional ultrasound and three-dimensional ultrasound transvaginal in the diagnosis of cearean scar pregnancy after cesarean section.Methods A retrospective analysis of 79 cases of two-dimensional and three-dimensional ultrasound images of CSP,comparison of two-dimensional ultrasound, two-dimensional combined three-dimensional ultrasound diagnosis of cearean scar pregnancy after cesarean section and Ultrasound diagnosis coincidence rate of different thickness of myometrium at the incision. Statistics are tested in chi-square test.Results In the cases of surgical diagnosis of incisional pregnancy after cesarean section, the coincidence rate of two-dimensional combined with three-dimensional ultrasound diagnosis was significantly higher than two-dimensional ultrasound diagnosis t(P〈0.05).when the thickness of myometrium is 〉2mm, the three-dimensional combined two-dimensional ultrasound diagnosis showed significantly higher phenotype in the coincidence rate compared with the two-dimensional ultrasound diagnosis (P〈0.05).Statistical analysis results indicated that the coincidence rate of two-dimensional combined three-dimensional ultrasound diagnosis was the same as the two-dimensional ultrasound diagnosis (P〉0.05) when the myometrium thickness is 〈2mm.Conclusion Two-dimensional combined three-dimensional ultrasound is more valuable than two-dimensional ultrasound diagnosis with the Myometrium thickness 〉2mm.
