针灸联合康复护理对老年中风后遗症患者生活质量的影响及护理效果观察

Effect of scupuncture combined with rehabilitation nursing on the quality of life of elderly patients with sequela of apoplexy and observation on nursing effect

目的探讨针灸与康复护理联合应用对老年中风后遗症患者生活质量的影响,并观察护理效果。方法择取2016年1月～2017年10月我院接收诊治的86例老年中风后遗症患者,随机分成两组,每组43例;对照组患者接受康复护理,观察组患者采取针灸联合康复护理的方式。护理2个月后,通过汉密尔顿抑郁(HAMD)量表、日常生活活动能力(ADL)量表以及神经功能缺损程度评分(NIHSS)量表评估两组患者康复效果;通过简明健康调查问卷(SF-36)评价两组患者护理前后生活质量变化。结果护理2个月后,两组患者HAMD、NIHSS评分均较护理前降低,ADL均较护理前升高,其中观察组HAMD、NIHSS评分明显低于对照组,ADL评分明显高于对照组,差异有统计学意义(P <0.05);两组患者SF-36评分明显高于护理前,且观察组SF-36评分高于对照组,差异有统计学意义(P <0.05)。结论中医针灸与康复护理联合应用能够有效改善老年中风后遗症患者抑郁情绪和神经缺损状态,提高患者生活能力,护理效果显著,且能有效提高患者生活质量,在老年中风后遗症患者中推广及应用价值均较高。

Objective To explore the effect of acupuncture and moxibustion combined with rehabilitation nursing on the quality of life of elderly patients with sequela of apoplexy and to observe the nursing effects. Methods 86 elderly patients with sequela of apoplexy who were admitted and treated in our hospital from January 2016 to October 2016 were selected and they were randomly divided into two groups according to admission order,with 43 cases in each group.Patients in the control group were given rehabilitation nursing awhile patients in the observation group were treated by acupuncture and moxibustion combined with rehabilitation nursing.After 2 months of nursing,rehabilitation effects of two groups were evaluated through the Hamilton depression （HAMD） scale,the daily living ability （ADL） scale and the nerve function defect scale （NIHSS） scale.The quality of life before and after the nursing of the two groups was evaluated by short-form-36 health survey （SF-36）. Results After 2 months of nursing,scores of HAMD and NIHSS were both lower than those before nursing while ADL score was higher than that before nursing.Scores of HAMD and NIHSS in the observation group were lower than those in the control group and the score of ADL in the observation group was higher than that in the control group.The difference was statistically significant （P 〈 0.05）. SF-36 Scores after nursing of two groups were both significantly higher than those before nursing.SF-36 Score of the observation group was higher than that of the control group.The difference was statistically significant （P 〈 0.05）. Conclusion Acupuncture and moxibustion combined with rehabilitation in treatment of patients with sequela of apoplexy can effectively improve their depression mood and nerve defect status and also improve patients＇ self-care ability,which has a significant nursing effect.In addition,it can effectively improve quality of life of patients,which is of high value of application and promotion in elderly patients with sequela of apoplexy.
