电视腹腔镜下子宫肌瘤剔除术临床分析及对患者免疫功能的影响

Clinical analysis of TV-assisted laparoscopic hysteromyomectomy and its impact on the immune function of patients

目的:分析与探讨将电视腹腔镜应用于子宫肌瘤剔除的价值及对临床疗效的影响与评价,并研究其对患者免疫功能的影响。方法:选择2015年3月～2017年3月经东方人民医院临床确诊后进行治疗的78例患者为临床研究病例,采用数字表法将其随机分为观察组和对照组。观察组39例患者采用电视腹腔镜下子宫肌瘤剔除术,对照组39例患者给予传统开腹子宫肌瘤剔除手术治疗。比较两组患者术中出血量、排气时间、进食时间、疼痛评分、下床时间和血清免疫球蛋白的变化。结果:观察组患者疼痛评分(3. 87±1. 91)分、术中出血量(61. 56±13. 33) ml、不良反应(2. 56%)、排气时间(27. 45±6. 04) h、Ig M(0. 86±0. 23) g/L、Ig G(8. 33±1. 92) g/L、Ig A(1. 98±0. 31) g/L、下床时间(6. 47±1. 54) h;对照组患者疼痛评分(5. 53±1. 64)分、术中出血量(102. 44±30. 47) ml、不良反应(20. 51%)、排气时间(39. 36±6. 81) h、Ig M(0. 63±0. 24) g/L、Ig G(7. 56±1. 04) g/L、Ig A(1. 76±0. 32) g/L、下床时间(14. 66±1. 23) h。观察组明显优于对照组,其差异具有统计学意义(P <0. 05)。结论:应用电视腹腔镜技术进行子宫肌瘤剔除术,临床治疗效果确切,手术创伤较小,显著减少了术中出血量和患者痛苦,明显减轻了对患者免疫功能的影响,有助于患者机体各项生理功能的较快恢复,满足了患者的期望值。

Objective ：To analyze the application value of TV-assisted laparoseopy in hysteromyomeetomy and its effect on the clinical curative effect, to study its impact on the immune function of patients. Methods ： 78 patients treated by Dongfang People＇s Hospital from March 2015 to March 2017 were selected and randomly divided into two groups by digital table method. 39 eases in the observation group were treated with TV-assisted laparoseopie hysteromyomeetomy while 39 eases in the control group were treated with traditional laparotomie hysteromyomeetomy. The changes in intraoperative blood loss, exhaust time, time of eating, pain score, time of getting out-of-bed and serum immunoglobulin were compared between the two groups. Results ： The pain score （ 3.87 ± 1.91 ） points, intraoperarive blood loss （61.56 ± 13.33） ml, incidence d adverse reaction （2.56%）, exhaust time （27. 45 ±6.04） h, the IgM （0.86 ±0.23） g/L, IgG （8.33 ±1.92） g/L, IgA （1.98 ±0.31） g/L, time d getting out- of-bed （6.47 ± 1.54） h were significantly better in the observation group than those in the control group, which were pain score （ 5. 53 ± 1.64 ） points, intraoperative blood loss （0. 63± 0. 24） g / L, IgG （ 7. 56 ±1.04 ） g / L, IgA （ 1.76 ±0. 32） g, exhaust time （ 39. 36 ± 6. 81 ） h, IgM （0. 63 ±0. 24） g / L, time of getting out-of-bed （ 14. 66 ± 1.23 ） h, with statistically significant difference （ P 〈 0. 05 ）. Conclusions ： TV-assisted laparoseopie hysteromyomeetomy is of exact curative effect and small surgical trauma, which can significantly reduce the intraoperative blood loss and pain and promote the recovery of physiological function, with little impact on the immune function of patients.
