不同治疗方式在凶险型前置胎盘患者中的效果、安全性及对性激素表达的影响研究

Study on the effect and safety of different treatment methods in the patients with pernicious placenta previa and influence for the expression of sex hormones

目的:观察与比较不同治疗方式在凶险型前置胎盘患者中的效果、安全性及对性激素表达的影响情况。方法:选取2015年8月至2017年4月成都市第五人民医院收治的40例凶险型前置胎盘患者为研究对象,将其根据治疗方式的不同分为A组(常规治疗组) 20例和B组(腹主动脉球囊阻断术组) 20例。统计与比较两组患者的手术时间、出血量、输血量、子宫切除率、并发症发生率及手术前后的VAS评分、血流动力学指标、创伤应激指标及性激素表达情况。结果:B组的手术时间短于A组,出血量及输血量均小于A组,子宫切除率及并发症发生率均低于A组,术后VAS评分低于A组,血流动力学指标好于A组,创伤应激指标低于A组,性激素水平表达情况好于A组,差异具有统计学意义(P <0. 05)。结论:腹主动脉球囊阻断术在凶险型前置胎盘患者中的效果好于常规治疗,安全性也更高,对患者性激素表达的不良影响较小。

Objective： To observe and compare the effect and safety of different treatment methods in the patients with pernicious placenta previa and influence situation for the expression of sex hormones. Methods： 40 patients with pernicious placenta previa in our hospital from August 2015 to April 2017 were selected as the study object,and they were divided into group A（ conventional treatment group） 20 cases and group B（ balloon occlusion of abdominal aorta group） 20 cases. Then the operation time,blood loss,blood transfusion volume,hysterectomy rates,complications rates,VAS score,hemodynamics,traumatic stress indexes and sex hormones expression situation before and after the operation of two groups were analyzed and compared. Results： The operation time of group B was shorter than that of group A,the blood loss and blood transfusion volume were less than those of group A,hysterectomy rate and complications rate were lower than those of group A,VAS score after the operation were lower than those of group A,hemodynamics indexes were better than those of group A,traumatic stress indexes were lower than those of group A,sex hormones expression situation were better than those of group A,the differences were all significant（ P〈0. 05）. Conclusions： The effect of balloon occlusion of abdominal aorta in the patients with pernicious placenta previa is better than those of conventional treatment,and the safety is higher,the bad influence for expression of sex hormones are smaller.
