PCR方法筛查无乳链球菌及预防性治疗的效果分析 被引量:2

摘要 目的:分析PCR方法对妊娠晚期妇女进行无乳链球菌筛查效果,并对无乳链球菌阳性病例进行跟踪,了解其预防性治疗效果。方法:对998例妊娠晚期妇女阴道-直肠标本进行PCR方法和显色培养法两种检测,比较两种方法间差异。选取2015年11月至2016年9月间无乳链球菌阳性患者563例,追踪调查直至分娩,统计经过预防性治疗后观察新生儿感染情况。统计25例新生儿感染GBS患者中母亲筛查与未筛查者比例。结果:在998例标本中PCR方法检测出阳性标本87例,阳性检出率为8. 70﹪,显色培养法检测出阳性标本61例,阳性检出率为6. 10﹪,PCR方法的阳性检出率高于显色培养法。经过追踪调查,发现在563例阳性标本中,进过产程过程中进行预防性治疗,新生儿被无乳链球菌感染的病例为1例,感染率为0. 17﹪。在25例新生儿感染GBS病例中,仅有一例为母亲有GBS感染史,其余母亲均未做过GBS筛查。结论:经PCR方法进行无乳链球菌筛查,阳性检出率明显高于显色培养法,通过筛查后对阳性病例进行预防性治疗,大大减低了无乳链球菌的对新生儿的致病率。所以使用PCR方法进行无乳链球菌筛查具有重要意义。 Objective: To analyze the effect of PCR method on the screening of Streptococcus agalactiae in the third trimester of pregnancy and to track the positive cases of Streptococcus agalactiae and to find out its preventive treatment effect. Methods: Two hundred and ninety-eight vaginal-rectal specimens from women of third trimester were detected by PCR and colorimetric methods,and the differences between the two methods were compared.From November 2015 to September 2016,563 cases of Streptococcus agalactiae positive patients were selected and traced until the delivery. The incidence of neonatal infection was observed after preventive treatment. Statistics of 25 cases of neonatal infection GBS patients with maternal screening and non-screening ratio. Results: There were 87 positive samples detected by PCR in 998 samples,the positive detection rate was 8. 7%. The positive samples were detected by chromogenic culture in 61 cases,the positive detection rate was 6. 1%. The positive detection rate of PCR method was high In color culture method. After follow-up investigation,found in 563 positive specimens,into the process of delivery of prophylactic treatment,neonatal infection with Streptococcus agalactiae in 1 case,the infection rate was 0. 17%. Of the 25 cases of neonatal GBS infection,only one case had a history of GBS infection in the mother and none of the remaining mothers had GBS screening. Conclusions: Streptococcus agalactiae screening by PCR method,the positive detection rate was significantly higher than the color culture method,after screening for positive cases of prophylactic treatment,greatly reducing the Streptococcus agalactiae of the newborn caused Disease rate. Therefore,the use of PCR for Streptococcus agalactiae screening is of great significance.
