探讨IGF-1、TGF-β1在宫腔粘连子宫内膜组织中的临床表达及意义 被引量:8

摘要 目的探讨IGF-1(胰岛素样生长因子-1)、TGF-β1(转化因子-β1)在宫腔粘连子宫内膜组织中的临床表达及意义。方法选择2014年1月~2017年1月期间在本院行宫腔粘连分离手术治疗的60例宫腔粘连患者作为观察组,按照患者宫腔粘连严重程度分为,轻度粘连组(n=20)、中度粘连组(n=20)、重度粘连组(n=20),并同期选择60例非宫腔粘连患者作为对照组,采用免疫组织化学法检测各组IGE-1、TGE-β1在子宫内膜组织中的临床表达,并分别比较IGE-1、TGE-β1临床表达与IUA分度关系及IGE-1、TGE-β1之间的相关性。结果 IGF-1、TGF-β1主要表达于子宫内膜腺上皮细胞胞质,少量表达于间质细胞,经阳性细胞计数统计,观察组IGF-1、TGF-β1阳性细胞百分率明显高于对照组(P<0.05),且重度粘连组IGF-1、TGF-β1阳性细胞百分率明显高于中度粘连组、轻度粘连组(P<0.05);经Spearman秩相关分析发现,IGF-1、TGF-β1阳性细胞百分率与IUA临床分度呈明显正相关性(P<0.05);经Pearson直线相关性分析,IGF-1、TGF-β1的阳性细胞百分率比较呈明显正相关(P<0.05)。结论 IGF-1、TGF-β1均参与了宫腔粘连的发病过程,均可临床作为评估宫腔粘连病情的重要指标。 Objective To investigate the clinical expression and signifcance of insulin-like growth factor-1 (IGF-1) and transforming factor-beta 1 (TGF-beta 1) in uterine adhesion endometrium. Methods 60 cases of intrauterine adhesions treated in our hospital from January 2014 to January 2017 were selected as the observation group.According to the severity of the adhesions, the patients were divided into the mild adhesion group (n=20), the moderate adhesion group (n=20) and the severe adhesion group (n=20), and 60 cases of non uterine cavity adhesion were selected at the same time. As the control group, the clinical expression of IGE-1 and TGE- beta 1 in endometrium was detected by immunohistochemistry. The relationship between the clinical expression of IGE-1, TGE- beta 1 and the degree of IUA, and the correlation between IGE-1 and TGE- beta 1 were compared.Results IGF-1 and TGF- beta 1 were mainly expressed in the cytoplasm of endometrium epithelial cells and expressed in a small amount in interstitial cells. The percentage of IGF-1 and TGF- beta 1 positive cells in the observation group was signifcantly higher than that in the control group (P〈 0.05). The percentage of IGF-1 and TGF- beta 1 positive cells in severe adhesions group was signifcantly higher than that in the moderate adhesion group and mild adhesion group, and the percentage of the positive cells was signifcantly higher than that in the moderate adhesion group. Adhesion group (P 〈 0.05);Spearman rank correlation analysis showed that the percentage of positive cells of IGF-1 and TGF-beta 1 was positively correlated with the clinical grading of IUA (P〈0.05).By Pearson linear correlation analysis, the percentage of positive cells of IGF-1 and TGF- beta 1 was positively correlated (P〈0.05). Conclusion Both IGF-1 and TGF-beta-1 are involved in the pathogenesis of intrauterine adhesions and can be used as important clinical indicators for evaluating the condition of intrauterine adhesions.
