2017年我院人血白蛋白临床使用情况分析

Analysis of clinical use of human albumin in our hospital in 2017

目的建立医院人血白蛋白临床使用评价标准,并对住院患者人血白蛋白的临床用药进行专项点评,为人血白蛋白合理使用提供参考。方法随机抽取2017年使用人血白蛋白治疗的住院患者972例,分析患者人血白蛋白使用的适应证、用法用量和配伍禁忌等。结果医院人血白蛋白用药合理616例,合理率为63. 37%;不合理用药356例,不合理率为36. 63%,主要表现为血清蛋白≥25 g/L 200例(56. 18%),病程中未记录用药原因72例(20. 22%),当作营养补充剂32例(8. 99%),用法用量不合理28例(7. 87%),其他24例(6. 74%)。结论医院人血白蛋白使用存在不合理情况,需加强管理,在临床实践中探索和积累合理使用白蛋白的循证医学证据,规范人血白蛋白的合理使用。

Objective To establish a clinical evaluation standard for human serum albumin in hospitals,and to give special comments on the clinical use of human serum albumin in hospitalized patients, and provide reference for the rational use of human serum albumin. Methods 972 inpatients treated with human albumin in 2017 were randomly selected, the indications,usage and incompatibility of the patient＇s human albumin use were analyzed. Results The hospital human serum albu min medication rate was 63.37% （616 cases） ,unreasonable medication a total of 356 cases（36.63% ） ,mainly for the serum protein is not less than 25 g/L 200 cases （56.18%） ,during the course of the disease 72 cases（20.22% ） did not record the cause of medication, as a nutritional supplement in 32 cases （ 8.99% ）, usage and dosage unreasonable in 28 cases （ 7.87% ） and other 24 cases （6.74%）. Conclusion The use of human albumin in the hospital is unreasonable, need to strengthen management, exploration and accumulation of evidence-based evidence for proper use of albumin in clinical practice, regulate the rational use of human albumin.
