游离第二趾胫侧血流桥接皮瓣移植覆盖术应用于手指掌侧皮肤损伤合并指动脉缺损患者的效果

Effect of free second toe tibial blood flow bridging flap graft covering on palm skin injury combined with finger artery defect

目的 分析游离第二趾胫侧血流桥接皮瓣修复伴指动脉缺损的手指创面的效果.方法 随机将洛阳正骨医院（郑州院区）2013年8月至2017年6月收治的81例手指掌侧皮肤损伤合并指动脉缺损患者分为两组,对照组40例行邻指动脉岛状皮瓣血流桥接修复术,观察组41例采取游离第二趾胫侧血流桥接皮瓣移植覆盖术,观察两组患者术后治疗效果、皮瓣存活率、分辨觉以及并发症等情况.结果 两组效果良好率、指间关节伸屈功能总主动活动度（TAM）/健侧值、整体外观评分以及并发症情况比较差异未见统计学意义（P〉0.05）.观察组分辨觉略高于对照组,差异有统计学意义（P〈0.05）.结论 游离第二趾胫侧血流桥接皮瓣用于修复伴指动脉缺损的手指创面有利于患者分辨觉的恢复,临床应用效果良好.

Objective To analyze the effect of free second toe tibial blood flow bridging flap on repairing finger wound with finger artery defect .Methods Eighty-one cases of finger palmar skin injury combined with finger artery defect in Luoyang Orthopedics Hospital from August 2013 to June 2017 were randomly divided into two groups .The 40 cases in control group were treated with adjacent digital artery island flap graft repair , and the 41 cases in observation group were treated with free second toe flap grafting flap grafting .The treatment effects , the survival rates of the flap , and the resolution of the flap of the two groups were observed .Results There was no significant difference in the good effect rate , TAM/side value , overall appearance score or the complications between the two groups （ P〉0.05 ） .The discrimination sense in observation group was higher than that in control group （ P〈0.05 ） . Conclusions The free second toe tibial blood flow bridge flap is used to repair the finger wound of the finger artery defect , which is beneficial to the recovery of the patient ＇s discrimination , and the clinical effect is good .
