乳腺超声造影、弹性成像技术单独及联合诊断乳腺癌的临床价值 被引量:4

Clinical value of single contrast-enhanced ultrasound, ultrasonic elastography and combined detection of the two methods in diagnosis of breast cancer

摘要 目的 分析乳腺超声造影、弹性成像技术单独及联合诊断乳腺癌的临床价值.方法 将2017年1至2017年12月于南阳市中医院行乳腺肿块切除治疗的108例患者(137个乳腺肿块)作为研究对象.入选者均接受乳腺超声造影、弹性成像技术检查.观察超声造影、弹性成像技术诊断结果,并以病理诊断结果作为金标准,计算超声造影、弹性成像技术诊断乳腺癌的灵敏度、特异度与准确度.结果 经病理检查证实137个乳腺肿块中恶性肿块、良性肿块各48个、89个.以病理检查结果作为金标准,超声造影诊断乳腺肿块灵敏度、特异度与准确度分别为79.17%、80.90%、80.29%;弹性成像诊断乳腺肿块灵敏度、特异度与准确度分别为77.08%、82.02%、80.29%;超声造影、弹性成像联合诊断乳腺肿块灵敏度、特异度与准确度分别为95.83%、92.13%、92.75%.超声造影、弹性成像技术检测乳腺肿块灵敏度、特异度与准确度比较,差异未见统计学意义(P〉0.05);较超声造影、弹性成像技术检查相比,超声造影、弹性成像技术联合检测乳腺肿块灵敏度、特异度与准确度相对较高,差异有统计学意义(P〈0.05).结论 乳腺超声造影、弹性成像技术在乳腺癌诊断中具有一定应用的价值,但将其联合应用有助于提升乳腺癌的鉴别诊断效能. Objective To analyze the clinical value of single contrast-enhanced ultrasound or combined with ultrasonic elastography in diagnosis of breast cancer .Methods A total of 108 patients with breast mass resection (137 breast masses ) in Nanyang Hospital of Traditional Chinese Medicine from January 2017 to December 2017 were selected , and they were given contrast-enhanced ultrasound and ultrasonic elastography examination .The diagnostic results of contrast-enhanced ultrasound and ultrasonic elastography were observed .The pathological diagnosis results were taken as the gold standard .The sensitivity , specificity , accuracy of contrast-enhanced ultrasound and ultrasonic elastography in diagnosis of breast cancer were recorded .Results Confirmed by pathological diagnosis , there were 48 malignant masses and 89 benign masses in 137 breast masses .The pathological results were taken as the gold standard , the sensitivity , specificity , accuracy of contrast-enhanced ultrasound in diagnosis of breast mass were 79.17%, 80.90%, 80.29%.The sensitivity , specificity , accuracy of ultrasonic elastography in diagnosis of breast mass were 77.08%, 82.02%, 80.29%.The sensitivity , specificity , accuracy ofcontrast-enhanced ultrasound combined with ultrasonic elastography in diagnosis of breast mass were 95.83%, 92.13%, 92.75%. There was no significant difference in the sensitivity , specificity , accuracy between contrast-enhanced ultrasound and ultrasonic elastography ( P〉0.05 ) .Compared with single contrast-enhanced ultrasound or ultrasonic elastography , the sensitivity , specificity , accuracy of contrast-enhanced ultrasound combined with ultrasonic elastography in diagnosis of breast mass were higher ( P〈0.05 ) .Conclusions Contrast-enhanced ultrasound and ultrasonic elastography in the diagnosis of breast cancer have certain value , and the combined application can increase the diagnostic efficiency for breast cancer .
