MRI与超声诊断早产儿脑损伤的对比分析

Comparative analysis of MRI and US in diagnosing brain injury in premature infants

目的 :分析MRI及超声对早产儿脑损伤(BIPI)的诊断价值。方法 :收集130例临床拟诊BIPI患儿,分别行超声和MRI检查,对比分析2种方法的检出率。结果:MRI共检出99例(76.2%),超声检出70例(53.8%),MRI阳性检出率明显高于超声(P<0.01)。其中MRI对脑白质损伤(WMD)、硬膜下出血(SD)及蛛网膜下腔出血(SAH)的检出率明显优于超声(均P<0.01),而超声对脑室周围-脑室内出血的检出率明显优于MRI(P<0.01)。结论:超声具有操作方便、价格低廉的优点,可作为BIPI初诊筛查的首选方法,但对WMD、SD及SAH的诊断逊于MRI。对于临床怀疑BIPI但超声检查阴性者应结合MRI以免漏诊。

Objective：To analyze the diagnostic value of MRI and ultrasound （US） on brain injury in premature infants （BIPt）. Methods ：130 cases with BIPI were collected,detected with US and MRI respectively,and the differences in the detection rate of brain injury in premature infants were compared and analyzed. Results：99 cases （76.2%） were positive in MRI,70 （53.8%） were positive in ultrasound,and the positive rate of MRI was significantly higher than that of US （P〈0.01）. MRI of brain white matter lesions （WMD）,subdural hemorrhage （SD） and detection of subarachnoid hemorrhage （SAH） was superior to US （P〈0.01） ,while the US around the ventricles-intraventricular hemorrhage （PIVH） detection was superior to MRI （P〈0.01）. Conclusions：US has the advantages of convenient inspection and low cost,which can be used as the preferred method of BIPt initial screening,but the diagnosis of WMD,SD and SAH is inferior to MRI. For patients with clinical suspicion of BIPI,we should combine MRI to avoid missed diagnosis.
