双源CT下肢动脉成像对比剂用量个体化注射方案的研究

Study on the individualized injection of contrast agent in the dual source CT angiography of lower extremity artery

目的:探讨双源CT下肢动脉成像对比剂个体化注射方案的临床意义。方法 :前瞻性收集60例行双源CT下肢动脉成像的患者,随机分为A组(常规对比剂注射方案组)、B组(个体化对比剂注射方案组),每组各30例。记录2组下肢动脉各监测血管分段的CT值及对比剂用量,由2名具有5年以上血管诊断经验的影像医师对图像质量分别进行主观及客观评价,并统计分析。结果:60例均获得了较满意的图像,B组主观影像评分明显优于A组(χ2=-2.181,P=0.027);2组下肢动脉各相同检测段血管的CT值均未见明显差异(均P> 0.05);B组对比剂用量少于A组。结论:双源CT下肢动脉成像对比剂用量的个体化方案可明显降低对比剂用量,且图像质量得到改善,可常规用于临床。

Objective：To discuss the clinical significance of the individualized injection of contrast agent in the dual source CT angiography of the lower extremities, nethods：A total of 60 patients with lower extremity arterial imaging with dual source CT were prospectively collected and randomly divided into group A （conventional contrast injection regimen group） and group B （individualized contrast medium injection scheme group）,30 cases in each group. The CT values and the dosage of contrast a- gents were recorded in two groups of arteries of the lower extremities. Subjective evaluation and objective evaluation of image quality were performed by two imaging doctors with more than 5 years＇ experience in vascular diagnosis,and the results were statistically analyzed. Results： Of the 60 patients who had been examined,30 of the A and B groups obtained satisfactory im- ages,the subjective image score of the individualized injection group was significantly better than that of the traditional injection group （X2=-2.181,P=-0.027）,and there was no significant difference in the CT value between the two groups （P〉0.05）,and the dosage of contrast agent in group B was less than that in group A. Conclusions ：In the dual source CT imaging of lower limb artery CTA,the individuation of contrast agent dosage can significantly reduce the dosage of contrast agent and the burden of kidney while ensuring the clinical diagnosis,which can be routinely used for clinical diagnosis and postoperative reexamination.
