摘要 狼疮性心肌炎(LM)是免疫复合物介导的心肌组织炎症反应。临床症状不典型,缺乏可靠的非侵袭性诊断方法。心脏核磁共振(CMR)作为评估疑似LM的一种基本诊断工具,具有无创伤性、无放射性、无需碘类对比剂、可重复性高的优势。新型的定量成像技术T1和T2-Mapping是识别心肌炎症、水肿和纤维化病变的敏感技术。CMR对LM的早期诊断、风险评估、指导治疗策略和预后评估有着重要价值。 Lupus myocarditis (LM) is characterized by complex myocardial autoimmune processes leading to inflammation, cardiomyocyte injury and reparative fibrosis.Clinical recognition of lupus myocarditis is challenging due to a paucity of classic cardiac symptoms.There is a lack of a reliable non-invasive diagnostic test. Cardiac magnetic resonance imaging (CMR) is a diagnostic tool for the assessment of patients with suspected lupus myocarditis. CMR has the advantages of non-traumatic, non-radioactive, no iodine contrast agent, high repeatability. T1 and T2-Mapping are new quantitative imaging techniques, which are sensitive techniques for identifying myocarditis, edema and fibrosis. CMR has important value for early diagnosis, risk assessment, guiding treatment strategy and prognostic evaluation of LM.
