摘要
目的讨论缺血性卒中患者血清NSE、BDNF水平变化与认知功能之间的关系。方法将2017年3月1日-2018年3月1日收治的60例缺血性卒中患者纳入本研究中,均使用简易智能状态量表(MMSE)对其进行评估,根据评估结果分为认知障碍组(30例)与非认知障碍组(30例),其中认知障碍组根据认知障碍程度分为A组(10例,轻度认知障碍)、B组(8例,中度认知障碍)及C组(12例,重度认知障碍),对其NSE、BDNF水平及认知功能相关性进行分析。结果治疗前认知功能障碍组患者NSE、BDNF水平均高于非认知功能障碍组患者(P<0.05,t=4.739、5.000),且治疗后两组患者NSE、BDNF水平均高于治疗前(P<0.05,t=5.421、5.321、7.211、8.304);在治疗前非认知功能障碍组患者ADL评分均高于认知障碍组患者(P<0.05,t=5.616);在治疗后两组患者ADL评分均高于治疗前(P<0.05,t=11.531、11.905);在治疗前NSE及BDNF水平B组均高于A组(P<0.05,t=4.074、7.474);C组均高于B组与A组(P<0.05,t=4.111、8.329、12.400、8.142);在治疗后B组NES水平高于C组(P<0.05,t=4.108);C组患者NES水平高于A组(P<0.05,t=4.529);在治疗后A组BDNF高于B组与C组(P<0.05,t=5.333、5.848);在治疗前B组ADL评分低于A组(P<0.05,t=4.286);C组ADL评分低于A组与B组(P<0.05,t=9.111、4.737);在治疗后B组ADL评分低于A组(P<0.05,t=4.486);C组ADL评分低于A组与B组(P<0.05,t=4.615、4.174);认知障碍组与非认知障碍组血清NSE及MMSE评分呈正相关(P<0.05,r=0.635、0.335)。结论缺血性卒中患者血清NSE及BDNF水平变化与认知功能呈负相关的关系。
Objective To investigate the relationship between serum NSE and BDNF levels and cognitive function in patients with ischemic stroke. Methods 60 patients with ischemic disease were enrolled in the study. The time of treatment was from March 1, 2017 to March 1, 2018. All patients were assessed using a mental status scale and were divided into cognitive impairments based on the evaluation results. In group(30 cases) and non-cognitive disorder group(30 cases), the cognitive impairment group was divided into group A(10 cases, mild cognitive impairment), group B(8 cases, moderate degree) according to the degree of cognitive impairment. Knowledge disorder and group C(12 cases, severe cognitive impairment), analyzed the relationship between NSE, BDNF levels and cognitive function. Results(1)In patients with pre-treatment cognitive dysfunction, the mean water level of NSE and BDNF was higher than that of patients with non-cognitive dysfunction, and the data was significantly different(P〈0.05, t = 4.739, 5.000); In the two groups after treatment, the mean water level of NSE and BDNF was higher than before treatment, and the data was significantly different(P〈0.05, t = 5.421、5.321、7.211、8.304).(2)The ADL score was higher than that of the cognitive impairment group before treatment(P〈0.05, t= 11.531、11.905). The ADL score was higher than that before treatment in both groups(P〈0.05,t= 6.173).(3)Both NSE and BDNF levels in group B were higher than Group A before treatment(P〈0.05, t = 4.074, 7.474); Group C was higher than Group B and Group A(P〈0.05, t = 4.111, 8.329, 12.400, 8.142); After treatment, the level of NES in group B was higher than that in group C(P〈0.05, t = 4.108); The NES level of patients in group C was higher than that of group A(P〈0.05, t = 4.529); After treatment, group A BDNF was higher than groups B and C(P〈0.05, t = 5.333, 5.848).(4)The ADL score in Group B was lower than Group A before treatment(P〈0.05, t = 4.286); The ADL score in Group C was lower than Group A and Group B(P〈0.05, t = 9.111, 4.737); After treatment, the ADL score in Group B was lower than Group A(P〈0.05, t = 4.486); The ADL score in Group C was lower than Group A and Group B(P〈0.05, t = 4.615, 4.174).(5) The serum NSE and MMSE scores were positively correlated with those of non-cognitive disorders(P〈0.05, r = 0.635, 0.335).Conclusions There was a negative correlation between the changes of serum NSE and BDNF levels and cognitive function in ischemic stroke patients.
