医院-社区-家庭模式延续护理对社区高血压人群应用效果

Application of continuing care of hospital-community-family model in community hypertensive population

目的:探讨延续护理在社区高血压人群中的应用及效果评价。方法:选取100例2016年1月至2016年12月佛山市高明区人民医院所属中山社区中的高血压患者,随机分为对照组和观察组,对照组采用常规护理,观察组在常规护理的基础上予以延续护理,出院后6个月对患者的健康行为、血压控制情况及护理依从性和满意度进行评估。结果:两组患者健康行为的调查中,在健康责任、自我实现、营养、人际关系、压力应对和运动6个方面的差异均有统计学意义,观察组得分显著高于对照组,组间比较,差异具有统计学意义(P <0.05);观察组患者的血压控制优于对照组,组间比较,差异具有统计学意义(P <0.05);观察组患者依从性和满意度均显著优于对照组,组间比较,差异具有统计学意义(P <0.05)。结论:延续护理能够帮助患者建立健康的生活行为,不仅可以促进患者有效控制血压,提高患者依从性和对护理的满意度,还能减少其他危险因素的发生。

Objective To explore the application of continue nursing on the hypertension crowd in the community. Methods 100 hypertension patients from Gaoming district people＇s hospital in the community were randomly divided into control group and intervention group from January 2016 to December 2016. The control group received routine nursing care, while the intervention group was given continuing nursing care on the basis of routine nursing.Assessing the health behavior, blood pressure control, nursing compliance and satisfaction of patients who discharged from hospital after 6 months. Results According to the survey of two patient groups, significant differences were observed in the six aspects-health responsibility, self-realization, nutrition, interpersonal relationships, stress coping and exercise.The scores of intervention group is higher than control groups（P〈0.05）. The blood pressure control of the intervention group is better than that of the control group（P〈0.05）. The compliance and satisfaction of the intervention group are obviously better than those of the control group（P〈0.05）. Conclusion Continue nursing which helps the patients establish a healthy lifestyle can not only promote the validation control of blood pressure, patient compliance and nursing satisfaction, but also reduce the occurrence of other hazards.
