同型半胱氨酸、血脂在糖尿病引发肾功能不全患者中的临床价值 被引量:3

摘要 目的对糖尿病引发的肾功能不全患者的同型半胱氨酸、血脂、血糖等生化指标进行分析探讨,分析这些指标在糖尿病引发的肾功能不全受损程度的临床诊断价值。方法收集64例体检健康人群作为对照组和180例由糖尿病引发的肾功能不全患者作为实验组,其中实验组包括肾功能不全代偿期60例,肾功能不全失代偿期60例,肾功能衰竭60例,检测其血清同型半胱氨酸(Hcy)、血脂(甘油三脂、胆固醇、高密度脂蛋白、低密度脂蛋白)、空腹血糖及肾功能指标(尿素氮、肌酐等生化指标,比较肾功能损伤程度不同期各项生化指标的变化。结果肾功能不全各期空腹血糖差异无统计学意义(P>0.05),而同型半胱氨酸水平平均比对照组明显升高,并且随着肾损伤程度的发展,血清同型半胱氨酸水平呈升高趋势,比较差异具有统计学意义(P <0.05)。血脂水平:肾功能不全代偿期和失代偿期的甘油三脂水平比对照组升高,具有统计学意义(P <0.05),而肾功能衰竭期的甘油三脂、胆固醇、高密度脂蛋白、低密度脂蛋白水平与对照组比较差异无统计学意义(P>0.05)。结论血清同型半胱氨酸、血脂水平的变化与糖尿病引发的肾功能不全的肾损伤程度具有临床诊断价值。 Object lye diabetic patients with renal insufficiency caused by homo-cysteine,blood lipid, blood sugar and other biochemical indicators analysis,analysis of the indicators in diabetic renal insufficiency caused by the damaged degree of the clinical diagnostic value.Methods to collect 64 cases of medical health people as control group and 180 cases of patients with renal insufficiency caused by diabetes as experimental group, the experimental group including renal insufficiency compensatory phase of 60 cases,renal insufficiency decompensation period of 60 cases, 60 cases of acute renal failure,detect the serum homocysteine (Hcy),blood fat,cholesterol,triglycerides,h igh-density lipoprotein cholesterol (hdl-c),low density Hpoprotein),fasting blood glucose and renal function index (urea nitrogen,creatinine,and other biochemical indicators,compare renal damage degree change of biochemical index in different period.Resu Its renal insufficiency periods fasting glucose differences no statistical significance (P〉0.05),and homocysteine levels,on average, People obviously higher than the normal control group,and with the development of renal damage degree,serum homocysteine levels showed a trend of increase,comparative differences statistically significant (P〈0.05).Lipid levels:renal insufficiency compensation period and decompensation period of triglycerides level higher than the control group,with statistical significance(P〈0.05),and renalfailure period of triglyceride,cholesterol,high-density lipoprotein cholesterol (hdl-c),low density lipoprotein level there was no statistically significant difference compared with control group(P〉0.05).Conclusion the changes of serum homoeysteine,blood lipid level and renal impairment caused by diabetes have clinical diagnostic value.
