管氏耳病六聪穴配合天竺灸治疗神经性耳鸣疗效观察

Therapeutic Observation of Guan's Er Bing Liu Cong Points plus Bamboo-tube Moxibustion for Nervous Tinnitus

目的比较采用管氏耳病六聪穴配合天竺灸与药物治疗神经性耳鸣的疗效差异。方法将80例神经性耳鸣患者随机分为针灸组和药物组,每组40例。针灸组取管氏耳病六聪穴(翳聪、耳灵、听会、角孙、翳风、听宫),并在外耳部施以天竺灸,每日针灸1次;药物组给予口服甲钴胺片,每次20 mg,每日3次。两组均15 d为1个疗程,治疗1个疗程后比较两组临床疗效、耳鸣残疾评估量表(THI)及微循环指标。结果针灸组总有效率为95.0%,显著高于药物组的57.5%(P<0.05)。针灸组治疗后耳鸣残疾评估量表、微循环指标(流态、管襻、襻周积分及总积分)积分均明显低于药物组,差异有统计学意义(P<0.05)。结论管氏耳病六聪穴配合天竺灸治疗神经性耳鸣疗效确切,能明显缓解患者耳鸣症状,改善机体微循环状态。

Objective To compare the therapeutic efficacy between Guan’s Er Bing Liu Cong points （six specific points for improving hearing） plus bamboo-tube moxibustion and medication in the treatment of nervous tinnitus. Method Eighty patients were randomized into an acupuncture-moxibustion group and a medication group, with 40 cases in each group. The acupuncture-moxibustion group was intervened by treating Guan’s Er Bing Liu Cong points [Yicong （Extra）, Erling （Extra）, Tinghui （GB2）, Jiaosun （TE20）, Yifeng （TE17）, and Tinggong （SI19）] plus bamboo-tube moxibustion at external ear, once every day; the medication group was given oral administration of mecobalamin, 20 mg each time, 3 times a day. For the two groups, 15-day treatment was taken as a course. The clinical efficacy, Tinnitus Handicap Index （THI） and microcirculation indexes of the two groups were compared after 1 course of treatment. Result The total effective rate was 95.0% in the acupuncture-moxibustion group, significantly higher than 57.5% in the medication group （P〈0.05）. After the intervention, the scores of THI and microcirculation indexes （flow state, capillary loop and loop surrounding scores and total score） in the acupuncture-moxibustion group were lower than those in the medication group, and the differences were statistically significant （P〈0.05）. Conclusion Guan’s Er Bing Liu Cong points plus bamboo-tube moxibustion is effective in treating nervous tinnitus, and it can significantly mitigate the tinnitus symptoms and improve the microcirculation state of the body.
