镇痛散外敷辅助阿片类药物治疗癌性疼痛疗效观察

Observations on the Efficacy of Opioids plus External Application of Analgesic Powder in Treating Cancer Pain

目的观察镇痛散外敷辅助阿片类药物治疗癌性疼痛的临床疗效。方法将64例肿瘤伴有癌性疼痛患者随机分为观察组和对照组,每组32例,对照组采用阿片类药物予以止痛治疗,观察组在对照组基础上加以镇痛散外敷疼痛部位(阿是穴)。观察两组患者止痛疗效、生活质量改善及不良反应发生情况。结果观察组总有效率为93.8%,高于对照组的68.8%(P<0.05)。两组药物起效时间比较差异无统计学意义(P>0.05),两组疗效持续时间、睡眠增加时间比较,观察组高于对照组(P<0.05),两组爆发痛次数比较,观察组少于对照组(P<0.05)。两组治疗后KPS评分比治疗前均有所改善(P<0.05)。对照组不良反应发生率为90.6%,观察组为37.5%,两组比较差异具有统计学意义(P<0.05)。结论镇痛散外敷辅助阿片类药物治疗癌性疼痛疗效显著,癌痛患者生活质量得到明显改善,同时也显著降低单纯运用阿片类药物的主要不良反应。

Objective To investigate the clinical efficacy of opioids plus external application of analgesic powder in treating cancer pain. Method Sixty-four patients with cancer pain were randomized to observation and control groups, 32 cases each. The control group was treated with opioid for pain relief and the observation group, by external application of analgesic powder at the tender point （Ashi point） in addition. The analgesic effect, quality of life and adverse reactions were observed in the two groups of patients. Result The total efficacy rate was 93.8% in the observation group, which was higher than 68.8% in the control group （P〈0.05）. There was no statistically significant difference in the time for the medicine to produce the effect between the two groups （P〉0.05）. The duration of the therapeutic effect and increased sleep time were shorter and the frequency of breakthrough pain was lower in the control group than in the observation group （P〈0.05）. The adverse reaction rate was 90.6% in the control group and 37.5% in the observation group and there was a statistically significant difference between the two groups （P〈0.05）. Conclusion Opioids with the help of external application of analgesic powder has a marked therapeutic effect on cancer pain and improve the quality of life in patients with cancer pain. It also significantly reduce the main adverse reactions to opioids used alone.
