摘要 目的探讨手部内生软骨瘤的影像学特征,提高对本病的认识及诊断的准确性。方法回顾性分析30例经手术和病理穿刺证实的发生于手部的内生软骨瘤患者的影像学资料。结果 30例患者中病变单发21例,多发9例;共39个病灶。X线及CT表现:病灶呈膨胀性骨质破坏,可见囊状透亮区,其内可见骨嵴和钙化,边缘硬化,8个病灶合并病理性骨折。MRI表现:平扫T1WI呈低信号,T2WI呈不均匀的高信号,增强扫描后边缘强化。SPECT/CT表现:病变区呈异常的核素浓聚,以瘤体内的骨嵴和钙化边缘为明显的异常放射性核素浓聚为主。30例患者的影像资料均未显示骨膜反应及软组织肿块。结论发生于手部的内生软骨瘤影像学表现具有明显的特征性,明确的诊断可以为临床有效治疗提供重要保障。 Objective To investigate the imaging feature of hand endogenetic chondroma, improve the understanding and the diagnosis accuracy of this disease. Methods The imaging data of 30 cases of endogenetic chondroma confirmed by operation and biopsy were retrospectively analyzed. Results Among 30 cases,the lesions were solitary in 21 cases, multiple in 9 cases, and 39 lesions in 30 cases. X-ray and CT findings showed the lesions were expansive bone destruction, cystic and transparent area, bone crest and calcification, marginal sclerosis, and 8 lesions combined with pathological fracture. MRI findings showed plain scan T1WI was low signal, T2WI showed uneven high signal intensity, and enhanced scan. SPECT/CT findings showed abnormal concentration of radionuclide was found in the lesion area. Abnormal radionuclide concentration was found in the bone crest and calcification margin of the tumor. None of the 30 patients showed periosteal reaction and soft tissue mass. Conclusion The imaging findings of hand endogenetic chondroma have obvious characteristics. Clear diagnosis can provide an important guarantee for clinical effective treatment.
