单纯型乳腺黏液癌与纤维腺瘤的X线表现分析

Comparative analysis of imaging features of pure mucinous carcinoma and fibroadenoma of the breast

目的分析单纯型乳腺黏液癌与乳腺纤维腺瘤的X线影像学表现,以提高对两种疾病的认识和鉴别。方法回顾性分析经手术病理证实的单纯型乳腺黏液癌16例,纤维腺瘤19例,分析病灶的大小、形态、分叶、密度、边界、钙化及晕症,然后分别对二者密度、分叶及晕症作一比较分析,进行Fisher精确检验,以P<0.05为差异有统计学意义。结果 16例黏液癌中,病灶表现为边界清楚或大部分清楚的椭圆或类圆形肿块;15例见分叶;11例表现为高密度,5例表现为等密度;1例病灶周围见晕症;伴粗颗粒和点状钙化各1例。19例纤维腺瘤中,病灶均表现为边界大部分清楚的椭圆或类圆形肿块;3例有分叶;18例表现为等密度,1例为高密度;13例病灶周围见晕症;伴爆米花钙化2例,粗颗粒钙化3例。二者密度比较:χ2=15.538,P<0.05;分叶比较:χ2=21.133,P<0.05;晕症比较,χ2=13.988,P<0.05。结论单纯型乳腺黏液癌与乳腺纤维腺瘤在X线表现上有一定特征性表现,尤其在病灶密度、分叶及晕症的表现上具有较大差异性,二者均表现为边界较清的肿块影,黏液癌常见分叶,密度较高,周围晕症少见,而纤维腺瘤较少见分叶,呈等密度,周围晕症常见。

Objective To study the imaging features of pure mucinous carcinoma and fibroadenoma of the breast. Methods Imaging findings were retrospectively reviewed in 16 women with pure mucinous carcinoma and 19 women with fibroadenoma of the breast lesions by operation and pathology.Analyze size,shape,microlobulated margin, density, calcification and ＂menbranoid sign ＂of the lesions,and then comparatively analyze density, microlobulated mar- gin and ＂menbranoid sign ＂of pure mucinous carcinoma and fibroadenoma of the breast, fisher＇s exact test is used forstatistical analyze（P〈0.05） which has significant difference. Results For the mammogram of 16 cases with BMC, all showed mass-like lesions;15 cases with microlobulated margin; 11 cases with high dense, 5 cases with isodense; ＂men- branoid sign ＂can be seen in 1 case; 1 case with benign calcification. For the mammogram of 19 fibroadenoma of the breast cases,all showed mass-like lesions; 3 cases with microlobulated margin; 18 cases with isodense,1 case with high dense;＂menbranoid sign ＂can be seen in 13 cases; 5 cases with benign calcification. The density of tuo groups were compared as follows： χ2=15.538, P〈0.05. Microlobulated margin were compared as follows： χ2=21.133, P〈0.05. ＂men- branoid sign＂ were compared as follows：χ2=13.98, P〈0.05. Conclusion Pure mucinous carcinoma and fibroadenoma of the breast have characteristic imaging findings on mammogram, especially It has much difference on density, mi- crolobulated margin and ＂menbranoid sign ＂of lesions. Both showed mass-like lesions with well-defined margin, PBMC usually had microlobulated margin, high density, ＂menbranoid sign ＂was seldom seen. However, microlobulated margin was seldom seen in fibroadenoma of the breast, usually showed isodensity and ＂menbranoid sign ＂ can be seen.
