腹腔镜与开腹直肠癌根治术对患者肠功能恢复的影响

EFFECTS OF LAPAROSCOPIC AND OPEN RADICAL RESECTION OF RECTAL CANCER ON INTESTINAL FUNCTION RECOVERY OF RECTAL CANCER PATIENTS

目的:对比腹腔镜直肠癌根治术与开腹直肠癌根治术对直肠癌患者术后肠功能恢复的影响。方法:60例直肠癌患者根据手术方式分为腹腔镜组与开腹组各30例,开腹组采用传统开放式手术治疗,腹腔镜组采用腹腔镜直肠癌根治术治疗。对比两组患者肠功能恢复情况,包括肠鸣音恢复时间、排气时间、留置胃管时间、恢复进食时间;比较两组患者手术时间和住院时间;分别比较两组患者术前,术后第1周、第4周的体重和Karnofsky评分。结果:腹腔镜组患者术后肠鸣音恢复时间、排气时间、留置胃管时间、恢复进食时间、手术时间、住院时间均明显短于开腹组,差异具有统计学意义(P<0.05)。两组患者术前体重和Karnofsky评分对比,差异无统计学意义(P>0.05);术后第1周、第4周,腹腔镜组的体重和Karnofsky评分均明显高于开腹组,差异有统计学意义(P<0.05)。结论:腹腔镜直肠癌根治术后直肠癌患者的肠功能恢复情况明显优于传统开腹组,且腹腔镜手术能明显缩短手术时间、住院时间,提高患者体重和Karnofsky评分,值得临床推广应用。

Objective: To compare the effects of laparoscopic and open radical resection of rectal cancer on intestinal function recovery of rectal cancer patients. Methods: 60 rectal cancer patients were divided into laparoscope group and open group with 30 patients in each group according to operation method. The patients in open group were given traditional open radical resection of rectal cancer, the patients in laparoscope group were given laparoscopic radical resection of rectal cancer. The intestinal function recovery, operation time, hospital stays, body weight and Karnofsky score of patients in 2 groups were compared. Results: The bowel sounds recovery time, evacuation time, gastric catheter indwelling time, resumption of eating time, operation time and hospital stays of patients in laparoscope group were all obviously shorter than open group(P<0.05). There was no significant difference about weight and Karnofsky score before operation between the laparoscope group and open group(P>0.05). The body weight and Karnofsky score of patients in laparoscope group 1 week, 4 weeks after operation were all obviously higher than open group(P<0.05). Conclusions: The intestinal function recovery after laparoscopic radical resection of rectal cancer is obviously better than open radical resection of rectal cancer; Moreover, laparoscopic radical resection can shorten operation time and hospital stays, as well as improve body weight and Karnofsky score. So it is worthy of clinical application.
