Factors affecting the dimission of nurses in China:a meta-synthesis of qualitative studies

摘要 [目的]系统评价护士离职原因的质性研究,为护理管理者稳定护理队伍采取针对性措施提供研究依据。[方法]检索收集MedLine、Embase、Cochrane Library、中国知网(CNKI)和万方数据库关于护士离职原因的质性研究,检索时限均从建库到2017年8月。将文献按照澳大利亚JBI循证卫生保健中心质性研究质量评价标准进行评价,应用Meta整合方法将研究结果进行整合。[结果]共纳入12项研究,提炼出84个研究结果,将类似的结果汇合成4个新的类别,并最终形成2个整合结果。整合结果1:不满意的工作环境和不健全的管理体系,导致护士身心负荷过重;整合结果2:护士职业不被认可,工作性质影响家庭和婚姻生活。[结论]质性研究Meta整合结果显示护士离职原因主要为两方面:不满意的工作环境和不健全的管理体系,导致护士身心负荷过重;护士职业不被认可,工作性质影响家庭和婚姻生活。 Objective:To conduct a systematic review of qualitative research on reason of nurse dimission and provide the research basis for the nursing managers to take the targeted measures to carry out the targeted measures. Methods: The qualitative research on the reasons for nurses dimission were retrieved from Medline, Embase, Cochrane I.ibrary, China National Knowledge Infrastructure (CNKI) ,and Wanfang database from the establishment of the database to August 2017. The literatures were evaluated according to the Australian JBI Evidence- Based Health Care Center Quality Evaluation Standards,and the results were integrated using the Metaintegration method. Results:A total of 12 studies were included, and 84 research results were extracted. The similar results were corn bined into 4 new categories, and finally,2 integrated results were formed. Integration result 1 :unsatisfactory working environment and unsound management system,resulting in a physical and mental overload of nurses;integration result 2 :not accept of the occupation, and the nature of work affecting family and married life. Conclusions:Meta-integration results show that nurses dimission mainly be cause of two aspects:unsatisfactory working environment and unsound management system,which leads to the physical and mental o verload of nurses; not recognized of the occupation ,and the nature of work affects family and married life.
