PDCA循环方法在肾内科医嘱差错管理中的应用

Application of PDCA cycle in the management of errors of doctor's advice in nephrology department

目的探讨PDCA循环管理方法在肾内科医嘱差错管理中的应用。方法按照PDCA循环的计划、实施、检查和处置,对肾内科病区医嘱时间进行测算,同时对医嘱差错进行基线扫描,分析存在的问题及原因,探讨改进措施、检查和落实。结果医师医嘱时间全部在9∶30之前完成。临床药师干预前1个月与PDCA闭环后1个月医嘱的差错分别为56、29例次。医嘱问题的主要原因是新进人员缺少培训和专业知识不足。结论临床药师加入医嘱审核流程,在医嘱审核中发挥了重要作用,可降低用药错误风险,提高药学服务质量。

Objective To explore the application of PDCA cycle in the management of errors of doctor＇s advice in nephrology department. Methods The time of each doctor＇ s advice in nephrology department was measured,and the errors in doctor＇s advice were scanned,then the causes of the problems,improvement measure,examination and implementation were analyzed according to the four steps of PDCA cycle,which included plan,implementation,check and disposal. Results All the medication orders were finished before 9∶ 30. There were 56 and 29 cases of errors respectively at 1 month before intervention of clinical pharmacists and after closing PDCA. The main reasons for the problems of doctor＇ s advice included lack of training and professional knowledge. Conclusion Clinical pharmacist plays an important role in the procedure of doctor＇s advice checking,which can reduce the risk of medication error and improve the quality of pharmaceutical care.
