摘要
目的探讨经会阴超声检查对女性压力性尿失禁的诊断价值。方法选取我院女性压力性尿失禁患者80例,按病情程度将患者分为轻度组(38例)和中重度组(42例);同时选取40例同期因健康查体就诊后排除女性压力性尿失禁及盆腔器官脱垂者为对照组。所有研究对象均行经会阴超声检查,对比分析检查结果。结果轻度组及中重度组患者膀胱尿道后角(α)、膀胱颈旋转角度(θ)、膀胱颈移动度(Mu)、膀胱颈下移距离(X)均高于对照组,且中重度组α角、θ角、Mu值高于轻度组,差异有统计学意义(P<0.05);α角、θ角、Mu值与压力性尿失禁程度均呈正相关(P<0.05),但X值与压力性尿失禁程度无明显相关性(P>0.05)。结论经会阴超声检查在女性压力性尿失禁患者中具有较高的价值,可清晰直观地对盆底解剖结构及功能改变情况进行观察,可为临床诊疗提供参考,值得推广应用。
Objective To explore the diagnostic value of perineal ultrasonography in female stress urinary incontinence.Methods Eighty female patients with stress urinary incontinence admitted to our hospital were divided into mild group (38cases) and moderate-to-severe group (42 cases) according to the severity of the disease. In the same period, 40 caseswomen without stress urinary incontinence and pelvic organ prolapse in our hospital due to physical examination wereselected into the control group. All research objects underwent perineal ultrasonography and the results of the examinationwere analyzed. Results The vesical urethra angle(α), bladder neck rotation angle(θ), bladder neck movement (Mu), bladderneck down distance (X) of the mild group and moderate-to-severe group were higher than those of the control group, andthe angle of α, θ, value of Mu, in the moderate to severe group were higher than the mild group(P〈0.05); and the angles ofα, θ and value of Mu were positively correlated with the degree of stress urinary incontinence(P〈0.05). but there was nosignificant correlation between the X and the degree of stress urinary incontinence (P 〉0.05). Conclusion Perinealultrasonography has high value in female patients with stress urinary incontinence. It can intuitively and clearly observe theanatomic structure and functional changes of the pelvic floor. It can provide reference for clinical diagnosis and treatment,which is worth promoting application.
