微创穿刺引流术对基底节中等量脑出血治疗效果研究

Study on effect of minimally invasive puncture drainage operation in treating moderate cerebral hemorrhage in basal ganglia

目的探讨微创穿刺引流术对基底节脑出血的临床治疗效果观察。方法通过随机抽签法选取该科2016年3月至2018年3月收治的60例基底节脑出血患者(血肿量30～50 m L)作为研究对象,并将其分为研究组和对照组,各30例。研究组采用微创穿刺血肿引流术治疗,对照组通过开颅血肿清除术治疗,比较2组患者的治疗效果。比较2组患者日常生活量表(ADL),Rankin量表(MRS)评分及并发症发生情况。结果 2组患者术后1、3个月ADL评分比较,差异均无统计学意义(P>0.05);研究组术后6个月ADL评分明显高于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。2组患者MRS评分比较,差异有统计学意义(P<0.05)。结论应用微创穿刺血肿清除术治疗基底节脑出血,操作简单、损伤小、手术安全性高,效果显著,不仅能提高患者的生活质量,还能有效地改善患者的预后,值得基层医疗单位神经外科推广。

Objective To observe the clinical effect of minimally invasive puncture drainage operation in treating mod-erate cerebral hemorrhage in basal ganglia. Methods Sixty patients with basal ganglia hemorrhage（hematoma volume 30-50 mL）in the neurosurgical department of the hospital from March 2016 to March 2018 were selected by using the random lotterymethod and divided into the control group and study group,30 cases in each group. The study group adopted the minimally inva-sive puncture drainage operation,while the control group was treated by craniotomy hematoma evacuation. Then the treatmenteffects were compared between the two groups. Results The ADL scores at postoperative 1,3 months had no statistical differ-ence between the two groups（P〉0.05）;the ADL score at postoperative 6 months in the study group was significantly higherthan that in the control group,and the difference was statistically significant（P〈0.05）. The occurrence rate of complications inthe study group was significantly lower than that in the control group with statistical difference（P〈0.05）.The MRS score hadstatistical difference between the two groups（P〈0.05）. Conclusion Using the minimally invasive hematoma evacuation fortreating basal ganglia cerebral hemorrhage has simple operation,small damage,high surgical safety,can obtain remarkable ef-fect,which not only can improve the quality of life in the patients,but also effectively improves the prognosis of the patients,and is worth promotion in the neurosurgery department of primary medical units.
