腹腔镜手术治疗T4期结肠癌进展

随着腔镜技术的发展,腹腔镜治疗结肠癌已经被认为是常规的治疗手段,但目前的NCCN指南对于局部周围组织器官浸润的T4期结肠癌尚不推荐行腹腔镜切除术。近年来,不少同行对T4期结肠癌的腹腔镜和开放手术进行回顾性分析,结果显示两个治疗组在手术安全性、肿瘤学结果及远期生存率方面没有明显差异。腹腔镜手术治疗T4期结肠癌对术者的手术技术有着较高要求,目前尚无前瞻随机对照研究和其他高级别循证依据来评价,腹腔镜手术在T4期结肠癌的治疗应用中依然处于探索阶段。本文就腹腔镜手术治疗T4期结肠癌的进展作一综述。

The development of laparoscopic technology has made it a regular application in treating colonic cancers. According to the National Comprehensive Cancer Network（NCCN） guidelines, however, laparoscopic operations are not recommended in treating T4 colonic cancer with local organ invasion. Researchers have compared laparoscopic operation and open approach in treating T4 colonic cancers in terms of operational safety, oncological outcome and long-terms survival rates, and the results indicate no significant difference. Since laparoscopic operation for T4 colonic cancer requires high levels of operational proficiency and lack support from evidence provided by prospective controlled randomized experiments, the adequacy of their clinical applications is still undermined. This article provides a conspectus of the development in laparoscopic treatment for T4 colonic cancers.
