“便利店”式社区居家整合照护体系之构建——基于台湾地区经验

Construction of “Convenience Store” Community-Home Integrated Care System Based on Taiwan’s Experience

中国台湾地区以“在地老化”（Aging in place）、“活跃老化”（Active aging）作为其长期照护体系的核心理念，贯彻“以老人为中心”的思想，为老人及身心功能障碍者提供“以社区居家照护为主，机构照护为辅”的“便利店”式社区整合照护服务。这一理念与大陆推行的“9064”养老服务模式存在异曲同工之处。通过理论分析社区居家整合照护体系的必要性，介绍台湾现行长照相关政策的发展历程、运作机制及经验教训，最后结合大陆社区居家养老服务模式以及现行的“医养融合”制度，对未来祖国大陆建设社区居家长期照护服务体系的发展进行思考和展望。

Taiwan Province of China now clearly defines ＂Aging in place＂ and ＂Active aging＂ as its core concept of the longterm care system. It implements the idea of ＂people -centered and elderlycentered care＂ and provides community integrated care services for the elderly and disabled people. This kind of care is like ＂convenience store＂ based on the principle of ＂community -based care at home same advantages with the ＂9064＂ elder service mode and supplemented by institutional care＂. This idea has the implemented by the mainland. This paper theoretically analyzes the necessity of integrating community - home care system and introduces the background, operation mechanism and experience of current policies in Taiwan. Finally, combining the existing problems of the community - home care model in China's Mainland with the existing long - term care insurance, we can think and look forward to the future development of China＇s community -home long- term care service system.
