围手术期快速康复护理干预在肱骨髁上骨折护理中的效果评价

Evaluation of the Effects of Nursing Intervention of Enhanced Recovery after Surgery during Perioperative Period for Supracondylar Fracture of Humerus

目的:评价围手术期快速康复护理干预在肱骨髁上骨折患儿中的护理效果。方法:将100例患儿按入院顺序分为观察组和对照组各50例,观察组给予快速康复模式的护理服务,对照组给予肱骨髁上骨折手术的常规护理。观察2组穿刺及拔针时的疼痛程度、一次性静脉穿刺成功率、焦虑评分及住院时间和住院费用。结果:术前穿刺及拔针时的疼痛程度观察组均轻于对照组,差异有统计学意义(P<0.05)。一次性静脉穿刺成功率观察组为96.0%(48/50),对照组为82.0%(41/50),2组比较差异有统计学意义(P<0.05)。2组焦虑评分在入准备室时、入手术室时、静脉穿刺时比较,差异均有统计学意义(P<0.05)。住院时间和住院费用2组比较,差异均有统计学意义(P<0.05)。结果:围手术期快速康复护理干预有助于减轻肱骨髁上骨折患儿疼痛,提高一次性静脉穿刺成功率,改善患儿焦虑状况,降低住院费用,缩短住院时间。

Objective： To assess nursing effects of nursing intervention of enhanced recovery after surgery（ERAS） during perioperative period for the children with supracondylar fracture of humerus. Methods： All 100children were randomized into the observation group and the control group according to the admission order, theobservation group were given nursing service of ERAS, while the control group conventional care for the surgery ofsupracondylar fracture of humerus. Hospitalization expense, hospitalization time, the anxiety conditions, the successrate of one-off veinpuncture and pain degrees when puncturing and withdrawing needles were observed in bothgroups. Results： The observation group was lighter than the control group in the pain degrees when puncturing beforethe surgery and withdrawing needles（P〈0.05）. The success rate of one-off veinpuncture in the observation groupwas 96.0% （48/50）, higher than 82.0% （41/50） of the control group, and the difference was statistically significant（P〈0.05）. The difference had statistical meaning in the comparisons of anxiety scores when entering preparationroom and surgical room and performing veinpuncture between both groups（P〈0.05）. The difference was statisticallysignificant in the comparisons of hospitalization time and hospitalization expenses between both groups（P〈0.05）.Conclusion： ERAS nursing intervention during perioperative period is helpful to mitigate the pain, raise the successrate of one-off veinpuncture, improve anxiety conditions of the patients, decrease hospitalization expenses and short-en hospitalization time.
