摘要
目的通过检测富含半胱氨酸的酸性分泌蛋白(SPARC)、Notch-1在乳腺癌组织中的表达水平,分析二者表达与疾病状态及临床预后之间的关系,为患者个体化治疗提供依据。方法选取潍坊市人民医院肿瘤内科就诊的70名乳腺癌患者及20名乳腺良性疾病患者为研究对象,采用免疫组化法检测组织中SPARC、Notch-1的表达,分析二者的表达与患者年龄、TNM分期、肿物大小、腋窝淋巴结转移情况、雌激素受体(ER)、孕酮受体(PR)、人表皮生长因子受体-2(Her-2)、Ki-67指数及其预后的关系。结果乳腺癌组织SPARC、Notch-1表达率明显高于乳腺良性组织(P<0.05);且二者表达与患者年龄、肿物大小、ER、PR等因素无关(P>0. 05); Notch-1表达与TNM分期、腋窝淋巴结转移、Her-2、Ki-67有关(P<0.05); SPARC表达与TNM分期、腋窝淋巴结转移有关(P<0. 05);SPARC、Notch-1表达对乳腺癌患者总生存时间(OS)无影响(P>0.05); Cox比例风险回归模型分析显示TNM分期、淋巴结转移、Her-2为复发的危险因子; TNM分期、淋巴结转移、组织学分级为死亡的危险因子。结论 SPARC、Notch-1在乳腺癌组织中的表达率明显高于乳腺良性肿瘤,且二者表达存在相关性;二者表达与患者TNM分期、腋窝淋巴结转移等临床病例指标相关,而与乳腺癌患者总生存率、复发及预后无显著关系。
Objective To detect the expression levels of cysteine-rich acidic secretion proteins (SPARC) and Notch-1 in breast cancer tissues,analyze the relationship between their expression and disease status and clinical prognosis,and provide basis for individualized treatment of patients. Methods 70 patients with breast cancer and 20 patients with benign breast disease were selected from the department of oncology.The expression of SPARC and Notch-1 in tissues was detected by immunohistochemistry.The relationship between expressions and disease status(age,TNM stage,tumor size,axillary lymph node metastasis,ER and PR,Her-2 state,and Ki-67 index) were analyzed. Results The expression rates of SPARC and Notch-1 in breast cancer tissues were significantly higher than those in benign breast tissues ( P 〈0.05).The differences were statistically significant, and the expression of SPARC and Notch-1 had no correlation with age,tumor size,ER,PR and other factors( P 〉0.05). The expression of Notch-1 was correlated with TNM stage,axillary lymph node metastasis,HER-2 and Ki-67( P 〈0.05).The expression of SPARC was correlated with TNM stage and axillary lymph node metastasis ( P 〈0.05).The SPARC and Notch-1 expression had no effect on the overall survival time (OS) of breast cancer patients ( P 〉0.05).Cox proportional hazards regression model analysis showed that TNM stage,axillary lymph node metastasis,and Her-2 were risk factors for recurrence.TNM stage,axillary lymph node metastatic and histological grades were risk factors for death. Conclusion The expression of SPARC and Notch-1 in breast cancer tissues was significantly higher than that in benign breast tumors,and the expression of them was correlated.The expression was related to the disease status of TNM stage and axillary lymph node metastasis,and had no significant relationship with the OS,recurrence and prognosis of breast cancer patients.
