摘要 目的探讨手机应用软件监测新生儿黄疸的可行性及临床意义。方法收集2017年1月~2018年6月我院产科出生新生儿及新生儿科患儿80例,对其依次采用自动拍照检测胆红素值(AIB)和新生儿总胆红素测定仪测定总胆红素值(TB),进行数据分析,比较AIB与TB的一致性,及其准确性和可行性。结果 80例足月儿及早产儿出生后1、3、5、7 d时AIB值与TB值比较,无明显差异(P>0.05);经Bland-Altman检验显示,AIB值与TB值有很强的一致性;线性回归分析显示,AIB值与TB值相关性r2=0.617,两者呈线性相关;AIB的ROC曲线下面积为0.768,灵敏度为81.6%,特异度为80.5%。结论 AIB与血清TB值的一致性良好,且具有较高灵敏度及特异度,可作为新生儿黄疸随访监测的一项方法,值得临床推广。 Objective To explore the feasibility and clinical significance of mobile application software for monitoring neonatal jaundice. Methods A total of 80 newborns who were born in the department of Obstetrics and 80 children in the department of Neonatology from January 2017 to June 2018 were enrolled in this study. Autophagia was used to detect bilirubin value (AIB) and neonatal total bilirubin tester was used to detect total bilirubin value (TB). The data was analyzed. The consistency, accuracy and feasibility of AIB and TB were compared. Results There was no significant difference in AIB value and TB value between the 80 full-term and preterm infants at the first, third, fifth and seventh days after birth(P〉0.05). The Bland-Altman test showed that the AIB value was consistent with the TB value. Linear regression analysis showed that the correlation between AIB value and TB value was r2=0.617, which was linearly correlated. The area under the ROC curve of AIB was 0.768, the sensitivity was 81.6%, and the specificity was 80.5%. Conclusion AIB has good consistency with serum TB, and has high sensitivity and specificity. It can be used as a method for follow-up monitoring of neonatal jaundice, which is worthy of clinical promotion.
