近端胃切除以及全胃切除治疗贲门癌的疗效比较

Comparison of Curative Effect of Proximal Gastrectomy and Total Gastrectomy in Treatment of Carcinoma of Gastric Cardia

目的探讨近端胃切除以及全胃切除治疗贲门癌的疗效。方法方便选择90例2016年1月—2018年1月贲门癌患者据术式分组。对照组开展近端胃切除手术治疗,观察组行全胃切除手术治疗。比较效果。结果观察组贲门癌手术干预效果44例(97.78%)高于对照组35例(77.78%)(χ~2=8.389,P<0.05);观察组手术出血总量、手术平均耗时、住院留观天数(86.02±10.12)mL、(76.01±13.61)min、(10.21±2.21)d和对照组(86.14±10.55)m L、(76.15±13.57)min、(10.62±2.59)d,差异无统计学意义(t=0.253、0.324、0.761,P>0.05);手术后观察组卡氏评分情况以及SF-36量表分值(84.87±6.11)分、(89.55±14.16)分优于对照组(74.21±5.25)分、(72.82±10.28)分,差异有统计学意义(P<0.05)。观察组吻合口瘘、肠梗阻、吻合口狭窄等并发症率4例(8.89%)低于对照组11例(24.44%),差异有统计学意义(χ~2=3.920 0,P<0.05)。结论贲门癌患者行全胃切除手术疗效非常理想,可有效改善患者病情,减少并发症的发生,促进患者生存质量改善。

Objective To study the curative effect of proximal gastrectomy and total gastrectomy in treatment of carcinoma of gastric cardia. Methods 90 cases of patients with carcinoma of gastric cardia admitted and treated in our hospital from January 2016 to January 2018 were convenient selected, the control group and the observation group were respectively treated with proximal gastrectomy and total gastrectomy, and the effect was compared between the two groups. Results The intervention effect in the observation group was higher than that in the control group, [44 cases（97.78%） vs 35 cases（77.78%）,（χ^2=8.389,P〈0.05）], and the total operative bleeding amount, average operation time, length of stay in the observation group and in the control group were respectively （86.02±10.12）mL,（76.01±13.61）min,（10.21±2.21）d,（86.14±10.55）mL,（76.15±13.57）min, （10.62±2.59）d,（t=0.253, 0.324, 0.761, P〉0.05）, and the karnofsky performance status and SF-36 scale score after surgery were better than those in the control group[（84.87±6.11）points,（89.55±14.16）points vs （74.21±5.25）points, （72.82±10.28）points（P〈0.05）, and the incidence rate of complications in the observation group was lower than that in the control group[4 cases（8.89%） vs 11 cases（ 24.44%）, χ^2=3.920 0,P〈0.05]. Conclusion The curative effect of total gastrectomy in treatment of carcinoma of gastric cardia is ideal, which can effectively improve the disease, reduce the occurrence of complications and promote the improvement of quality of life.
