鼻内镜下改良下鼻甲成形术对鼻窦炎伴肥厚性鼻炎患者主要症状评分及相关临床指标的的影响

Effect of Modified Inferior Turbinate Plasty under Nasal Endoscope on the Score of Major Symptoms and Related Clinical Indexes of Patients with Sinusitis with Hypertrophic Rhinitis

目的观察鼻内镜下改良下鼻甲成形术对鼻窦炎伴肥厚性鼻炎患者的症状及临床指标的影响。方法方便选取2011年11月—2015年10月间该院收治的100例鼻窦炎伴肥厚性鼻炎患者作为研究对象,随机分为实验组和对照组,实验组患者接受改良下鼻甲成形术,对照组患者接受下鼻甲部分切除术,术后随访6个月,记录并对比两组的临床指标、并发症及疗效。结果实验组总有效率(94.00%)高于对照组(72.00%),差异有统计学意义(χ~2=8.58,P=0.00)。实验组患者的手术时间、术中出血量、术后恢复通气时间均小于对照组患者,差异有统计学意义(t=6.64、8.23、2.02;P=0.00、0.00、0.00);实验组患者术后并发生总发生率(24%)低于对照组(66%),差异有统计学意义(χ~2=17.82;P=0.00)。结论鼻内镜下改良下鼻甲成形术应用于鼻窦炎伴肥厚性鼻炎患者,具有疗效好,出血少、恢复快、术后并发症少的优点,值得在临床推广应用。

Objective To observe the effect of modified inferior turbinate plasty under nasal endoscope on the score of major symptoms and related clinical indexes of patients with sinusitis with hypertrophic rhinitis. Methods 100 cases of patients with sinusitis with hypertrophic rhinitis admitted and treated in our hospital from November 2011 to October 2015 were convenient selected and randomly divided into two groups, the experimental group used the modified inferior turbinate plasty under nasal endoscope, while the control group used the partial turbinectomy, and the clinical indexes, complications and curative effect were recorded, by 6-month follow-up. Results The total effective rate in the experimental group was higher than that in the control group （94.00% vs 72.00%）, and the difference was statistically significant（χ^2=8.58, P=0.00）, and the operation time, intraoperative bleeding amount and postoperative ventilation time in the experimental group were lower than those in the control group, and the differences were statistically significant （t=6.64, 8.23, 2.02; P=0.00, 0.00,0.00）, and the total incidence rate of complications after surgery in the experimental group was lower than that in the control group （24% vs 66%, χ^2=17.82; P=0.00）, and the difference was obvious. Conclusion The curative effect of modified inferior turbinate plasty under nasal endoscope in treatment of patients with sinusitis with hypertrophic rhinitis is good, with less bleeding, rapid recovery and few complications after surgery, and it is worth clinical promotion and application.
