腹腔镜保留肾单位手术治疗T1期肾癌的疗效分析

Analysis of Curative Effect of Laparoscopic Nephron Sparing Surgery in Treatment of Renal Cancer in T1 Stage

目的探讨腹腔镜保留肾单位手术治疗T1期肾癌的疗效分析,为肾癌的临床治疗提供科学理论依据。方法方便选取该院2016年9月—2018年1月在该院治疗的164例T1期肾癌患者为研究对象,随机分为两组,分别为对照组和观察组,每组82例,其中对照组患者采用腹腔镜保留肾单位手术进行治疗,观察组患者采用肾癌根治术治疗,观察两组患者的手术时间、肾动脉阻断时间、术中出血量等情况。结果两组患者手术均顺利完成,其中观察组患者的平均手术时间为(172.43±30.37)min,出血量为(125.58±20.95)mL,术后留置引流管时间(5.64±11.95)d,术后住院(15.14±2.89)d,经统计学分析,与对照组相比,差异有统计学意义(t=1.498、-2.392、-1.294、-2.644, P=0.144、0.023、0.148、0.023)。结论腹腔镜保留肾单位手术治疗T1期肾癌,不仅治疗效果明显,还可以减少患者的痛苦,具有出血量少、创伤小、并发率低的优点,而且可以改善患者生活质量,安全性较高,值得在临床上应用推广。

Objective To discuss the curative effect of laparoscopic nephron sparing surgery in treatment of renal cancer in T1 stage thus providing scientific theoretical basis for the clinical treatment of renal cancer. Methods 164 cases of patients with renal cancer in T1 stage admitted and treated in our hospital from September 2016 to January 2018 were convenient selected and randomly divided into two groups with 82 cases in each, the control group were treated with laparoscopic nephron sparing surgery, while the observation group were treated with radical nephrectomy, and the operation time, renal artery occlusion time and intraoperative bleeding amount of the two groups were observed. Results The two groups had successfully finished the operation, and the average operation time, bleeding volume, postoperative drainage tube indwelling time, postoperative length of stay in the observation group were respectively （172.43±30.37）min, （125.58±20.95）mL, （5.64±11.95）d,（15.14±2.89）d, and and the differences were statistically significant（t=1.498,-2.392,-1.294,-2.644, P=0.144, 0.023, 0.148, 0.023）. Conclusion The effect of laparoscopic nephron sparing surgery in treatment of renal cancer in T1 stage is obvious, which can reduce the pains with less bleeding volume, small wound and low complication rate, and it can also improve the quality of life with higher safety, and it is worth clinical promotion and application.
