冠心病心绞痛血瘀证证候组成相关文献研究分析

Literature Analysis of the Composition of Blood Stasis Syndrome of Coronary Heart Disease and Angina Pectoris

目的：对冠心病心绞痛血瘀证证候组成的研究文献进行汇总、分析,为临床医师诊断提供可靠的参考。方法：检索CNKI、CBM、维普和万方数据库中1970—2016年发表的冠心病心绞痛血瘀证证候组成的相关文献,统计冠心病心绞痛疾病中血瘀证证型患者的分布比例,并对其相关文献所纳入的中医诊断标准进行比较分析,以及对每篇文献中有具体冠心病心绞痛血瘀证证候组成、出现频数及所占百分比进行统计分析。结果：共纳入期刊文献6篇。其中有3篇文献冠心病心绞痛血瘀证证型患者的比例分布〉 50%。文献中冠心病心绞痛中医辨证分型所参考的诊断标准分别有1990年中国中西医结合学会心血管专业委员会制定的冠心病中医辨证标准,1994年国家中医药管理局制定的中医内科疾病诊断疗效标准,1997年国家技术监督局发布的中医疾病诊断疗效标准（行业标准）及2002年国家食品药品监督管理局制定的中药新药临床研究指导原则,在纳入的这4个诊断标准中,共同的证候有：胸闷,心胸疼痛;痛有定处,固定不移;面色晦暗;舌质紫黯有瘀斑;脉结代5项。不同点则在冠心病心绞痛血瘀证疼痛的特点、伴随症等方面有了新认识。而在冠心病心绞痛血瘀证患者的证候组成、出现频数及所占总研究人数的百分比的统计分析中,百分比大于50%的证候有胸痛（心胸刺痛）、固定不移、胸闷、心悸、舌黯红、舌底青紫/舌下脉络紫、脉沉或涩。结论：冠心病心绞痛血瘀证相关文献证候组成的研究与目前已有《中华人民共和国中医药行业标准·中医病症诊断疗效标准》相符合,国家行业标准是最受中医医疗与教学的肯定,从而为临床医师提供更加明确的诊断标准。

Objective：To summarize and analyze the literatures about the syndrome composition of angina pectoris with blood-stasis syndrome,so as to provide reliable reference for clinicians. Methods：Retrieval from CNKI,CBM,VIP and WanFang database published from 1970 to 2016 coronary heart disease and angina of blood stasis syndrome consisting of literatures,to count the distribution proportion of blood stasis syndrome in patients with coronary heart disease and angina pectoris,and compare the traditional Chinese medical diagnostic standards included in related literatures,and analyze the composition,frequency and percentage of specific coronary heart disease,angina pectoris,blood stasis syndrome in each document. Results：Obtain 6 journal articles. Among them,there were 3 articles,the proportion of patients with blood stasis syndrome type 50% in the coronary heart disease,angina pectoris. In the literature,the angina pectoris of TCM syndrome differentiation,which is included in TCM diagnostic criteria,are the Chinese Medicine Association of Traditional Chinese and Western Medicine,Cardiovascular Specialized Committee formulated the standard of TCM coronary heart disease in 1990;the State Administration of Traditional Chinese Medicine（TCM） formulated the criteria for the diagnosis and treatment of internal diseases of Chinese medicine in 1994;the State Bureau of Technical Supervision promulgated the criteria for the diagnosis of diseases（industry standards） in 1997 and the guidelines for the clinical research of new drugs of traditional Chinese medicine formulated by the State Food and Drug Administration in 2002 respectively. In the 4 diagnostic criteria,the common syndromes are：chest tightness,chest pain;fixed pain,fixed do not move;dark complexion;dark purple tongue with ecchymosis;knotted pulse 5 items. Different points in coronary heart disease,angina pectoris,blood stasis syndrome,pain characteristics,accompanied by diseases and other aspects of the new understanding. In patients with blood-stasis syndrome of angina pectroris composition,frequency and percentage of the total number of accounts for the statistical analysis of the syndrome,percentage is more than 50% chest pain（heart pain）,fixed do not move,chest tightness,heart palpitations,dark red tongue,purple tongue,sublingual vein/purple veins or sink astringent. Conclusion：The literature research on syndrome of blood stasis syndrome of coronary heart disease and angina pectoris has satisfied with the present the People＇s Republic of China Traditional Chinese Medicine Industry·Standard of TCM Diagnosis and Treatment,the national profession standard is most affirmed by the medical treatment and teaching of traditional Chinese medicine,so as to provide more clear diagnostic criteria for clinicians.
