中药止血方对尿道前列腺电切术后出血临床疗效观察

Clinical Observation of Traditional Chinese Medicine Zhixue Decoction on Hemorrhage after Transurethral Resection of Prostate

目的：探讨中药止血方口服汤剂对经尿道前列腺电切术（TURP）后出血的治疗效果。方法：2015年1月—2017年6月行经尿道前列腺电切术的良性前列腺增生症患者75例,其中中药止血方治疗组37例,对照组38例。对照组常规预防感染、对症治疗,中药止血方治疗组口服中药止血方,两组患者术后均不用其他止血药物。术后对＂即刻、第1天、第3天、第7天＂4个时段尿管引流液颜色和尿隐血进行记录并比较。将尿管引流颜色分为＂鲜红、淡红、清亮/淡黄＂3个标准,尿常规隐血（BLU）分为＂-、1＋、2＋、3＋＂4个标准,血红蛋白下降幅度分为10 g/L以下、10～30 g/L、30 g/L以上。结果：两组患者术后即刻、术后第1天、术后第3天尿管引流液颜色比较无明显差异（P〉0.05）;术后第7天尿管颜色比较有显著性差异（P〈0.05）。尿常规隐血术后即刻、术后第1天比较无明显差异（P〉0.05）;术后第3天和第7天比较有显著差异（P〈0.05）;血红蛋白下降幅度术后即刻、术后第1天比较无明显差异（P〉0.05）;术后第3天和第7天比较有显著差异（P〈0.05）。结论：中药止血方口服汤剂减少经尿道前列腺电切术后创面出血,疗效显著,值得临床推广应用。

Objective：To investigate the curative effect of Traditional Chinese Medicine ZhixueDecoction on hemorrhage after transurethral resection of prostate（TURP）. Methods：From January 2015 to June 2017,75 cases of benign prostatic hyperplasia（BPH）treated by transurethral resection of prostate were included. Among them,37 cases were treated with traditional Chinese medicine for the treatment of hemostasis and 38 cases were as control. The control group was given routinely prevent infection,symptomatic treatment,the treatment group was given Traditional Chinese Medicine ZhixueDecoction stop bleeding,two groups of patients didn＇t given other hemostatic drugs after operation.Postoperative urinary exudate color and urinary occult blood were recorded and compared at four time points of＂immediately,the 1 st day,the 3 rd day and the 7 th day after operation＂. The standard of urinary drainage was divided into three standards：bright red,light red,clear/light yellow. Urinary routine occult blood（BLU）was divided into four criteria of＂-,1＋,2＋,3＋＂and hemoglobin decline was divided into 10 g/L or less,10-30 g/L,and 30 g/L or more. Results：There was no significant difference in the color of catheter between the two groups immediately and 1 st day,3 rd day after operation（P〉0.05）,but on the 7 th day after operation was significantly different（P〈0.05）. There was no significant difference in the BLU between the two groups immediately and 1 st day after operation（P〉0.05）,but on the 3 rd day and the 7 th day after operation was significantly different（P〈0.05）. The decrease of hemoglobin immediately and 1 st day after operation was no significant difference（P〉0.05）,but the 3 rd day and the 7 th day after operation was significantly different（P〈0.05）.Conclusion：Traditional Chinese Medicine Zhixue Decoction can reduce wound bleeding after TURP and the curative effect is significant. It is worthy of clinical application.
