针刺联合玻璃酸钠关节腔注射治疗膝关节骨关节炎临床疗效观察

Clinical Observation of Acupuncture Combined With Sodium Hyaluronate Injection in Treating Osteoarthritis of Knee Joint

目的：观察针刺联合玻璃酸钠注射液关节腔内注射治疗膝关节骨性关节炎（KOA）的临床疗效。方法：将73例KOA患者随机分为A组25例、B组24例及AB组24例。A组采用针刺疗法治疗,B组予玻璃酸钠注射液关节腔内注射治疗,AB组予针刺联合玻璃酸钠注射液关节腔内注射治疗。观察3组治疗前后Lysholm膝关节功能评分、视觉模拟评分（VAS）、JOA膝关节骨性关节炎功能评估,并行组内与组间比较。两组根据中医病证诊断疗效标准进行疗效评估。结果：治疗2周后及24周随访时,3组治疗后VAS评分、Lysholm评分、JOA膝骨性关节炎功能评估分数,较治疗前均有显著差异,经比较（P〈0.05）,有统计学意义;且AB组各项评分均高于A组和B组,差异均有统计学意义（P〈0.05）;A组和B组膝关节各项评分差异无统计学意义（P〉0.05）;AB临床治愈率95.8%,明显高于其他2组（80%、75%）。结论：针刺联合玻璃酸钠注射液关节腔内注射治疗可有效控制KOA患者病情,改善膝关节功能,提高了病人的生活质量,值得临床推广应用。

Objective：To observe the clinical efficacy of acupuncture combined with Sodium Hyaluronate Injection intraarticular injection in the treatment of knee osteoarthritis（KOA）. Methods：73 patients with KOA were randomly divided into A group（n=25）,B group（n=24）and AB group（n=24）. Group A was treated with acupuncture,group B was treated with Sodium Hyaluronate Injection intraarticular injection,group AB was treated with acupuncture combined with Sodium Hyaluronate Injection intraarticular injection. The Lysholm knee function score,visual analogue scale（VAS）and JOA knee osteoarthritis function evaluation were observed before and after treatment in the 3 groups,and the comparison between the parallel group and the group was also made. The two groups were evaluated according to the curative effect standard of TCM disease diagnosis. Results：after 2 weeks of treatment and followed up for 24 weeks,VAS scores and Lysholm score,JOA knee osteoarthritis 3 group after treatment than before treatment were significantly different,by comparison,was statistically significant（P〈0.05）;and the AB group were higher than that of A group and B group,the difference had statistical significance（P〈0.05）;There was no significant difference in knee score between group A and group B（P〉0.05）;AB clinical cure rate was 95.8%,significantly higher than the other 2 groups（80%,75%）. Conclusion：Acupuncture combined with intraarticular injection of Sodium Hyaluronate Injection can effectively control the condition of patients with KOA,improve the function of knee joint,and improve the quality of life of patients,which is worthy of clinical application.
