芪苈强心胶囊联合左卡尼汀对慢性充血性心力衰竭血清IL-6、IL-10及TNF-α影响

Effect of Qili Qiangxin Capsule Combined with Levocarnitine on Serum IL-6,IL-10 and TNF-α in Chronic Congestive Heart Failure

目的：探讨芪苈强心胶囊联合左卡尼汀对慢性充血性心力衰竭的临床疗效及其对血清IL-6、IL-10及TNF-α的影响。方法：采用病例对照研究的方法对2017年1月—2017年12月于我院住院的80例慢性充血性心力衰竭患者进行临床观察。将患者按随机数字表分配,实验组共40例,对照组40例,其中对照组患者在常规西医治疗的基础上使用左卡尼汀进行治疗,实验组患者则在常规西医治疗的基础上使用左卡尼汀联合芪苈强心胶囊进行治疗,两组患者均治疗1个月。治疗结束后计算临床总有效率,观察患者血清IL-6、IL-10及TNF-α的变化,中医症状积分及6 min步行距离。结果：对照组临床总有效率为77.50%,实验组临床总有效率为92.50%,实验组优于对照组（P〈0.05）;实验组血清IL-6、IL-10及TNF-α水平改善情况较对照组更为明显（P〈0.05）;治疗后实验组患者胸闷气喘、咳嗽、咯痰中医症状积分明显低于对照组（P〈0.05）;治疗组患者6 min步行距离较对照组增加明显（P〈0.05）。结论：芪苈强心胶囊具有益气温阳、活血通络、利水消肿的作用,芪苈强心胶囊联合左卡尼汀可以明显改善慢性充血性心力衰竭患者心功能情况,使患者血清IL-6、IL-10及TNF-α含量等指标得到明显改善,临床上值得应用。

Objective：To discuss Qili Qiangxin Capsule combined with levocarnitine on chronic congestive heart failure clinical effect of and its effect on serum IL-6,IL-10 and TNF-α. Methods：A case-control study was conducted in 80 hospitalized patients with chronic congestive heart failure in our hospital from January 2017 to December 2017. The patients were assigned on random number table,40 case in experimental group and 40 case in control group. The control group was treated by Levocarnitine,based on conventional Western medical treatment. The experimental group was treated by Levocarnitine,which was based on conventional Western medical treatment as well,however,Qili Qiangxin Capsule was combined with it. Both of them would continue for a mouth. The total clinical effective rate would be calculated. The changes of serum IL-6,IL-10 and TNF-α,symptom score of TCM and distance of 6 minutes walking would be observed after experiment. Results：The total clinical effective rate was 77.50% in the control group and 92.50% in the experimental group,which was better than control group（P〈0.05）. The levels of IL-6,IL-10 and TNF-α in experimental group was better than control group as well（P〈0.05）. The TCM symptom scores of chest tightness,asthma,cough and expectoration in the experimental group were significantly lower than those in the control group after experiment（P〈0.05）;The patients in the treatment group had a longer walking distance in 6 minutes than the control group had obviously（P〈0.05）. Conclusion：Qili Qiangxin Capsule has the effects of invigorating Qi and activating blood,promoting blood circulation,inducing diuresis to alleviate edema. Qili Qiangxin Capsule combined with levocarnitine can obviously improve the cardiac function in patients with chronic congestive heart failure,making the serum IL-6,IL-10 and TNF-α levels and other targets have been significantly improved,which means the clinical worth of application.
