临床路径管理在计划性剖宫产中的成本与效果分析

The analysis of the cost and effect of planned cesarean section under clinical pathway management

目的以某省级三甲医院的计划性剖宫产为研究对象,探讨临床路径管理系统对计划性剖宫产术的成本和效果的影响。方法采用案例对照方法。结果 (1)平均住院时间在实施临床路径后显著缩短,剖宫产成本控制效果良好,且药费下降明显(P<0.001),差异有统计学意义;(2)药占比由未实施临床路径前的29%下降为14%,降幅最大。结论剖宫产术采用临床路径管理系统后,产科的卫生资源得到更加高效的配置,项目成本比例也更加符合高质量的医疗投入,医院的经济效益得到大幅度提升。

Objective Taking the planned palace production of a provincial third class hospital as the object of study,to explore the impact of the clinical path management system on the cost and effect of planned cesarean section. Methods a case control method was used. Results The average hospitalization time was significantly shorter after the implementation of the clinical pathway,and the cost control effect of cesarean section was good. Thus,to make the costs under control and to decrease the drug cost obviously（ P〈0.001）. The drug proportion was reduced from 29% to 14%,and the lowered range was the biggest. Conclusions After the application of clinical pathway management system in cesarean section,the health resources of obstetrics have been more efficient,and the cost ratio of the project is more in line with the high quality medical input,and the economic benefit of the hospital has been greatly improved.
