产前超声不同部位强回声胎儿82例临床分析

Clinical analysis of 82 fetuses with strong echo foci in different parts of the fetus

目的分析产前超声检出不同部位强回声灶胎儿的超声声像特征及随访结果。方法回顾性分析产前超声检出胎儿不同部位强回声灶82例单胎妊娠孕妇的临床资料。结果 1例脑实质强回声灶,双侧侧脑室扩张明显,左侧脑实质回声增强,边界清晰;3例肝脏钙化,肝实质内可见点状强回声、声影不明显;3例腹腔强回声灶中2例胎儿腹腔钙化、肠管不同程度扩张,1例呈混合回声包块,可见长骨样结构及实性组织、囊腔等结构;75例心脏强回声灶中1例左心室侧壁中强回声团块,1例心脏腱索多发钙化,73例心脏乳头肌与腱索上的单一钙化(心室内强回声光点);失访10例(心脏强回声灶),终止妊娠5例(1例脑实质强回声灶,2例肝脏多发钙化灶,2例心脏强回声灶);3例腹腔强回声灶胎儿出生后行手术治疗,预后良好;64例随访钙化性病变消失(1例肝脏单发钙化,63例心脏强回声)。结论胎儿不同部位强回声灶超声声像存在差异,多数妊娠结局良好。

Objective To analyze the prenatal ultrasonographic features and clinical pregnancy outcomes of 82 fetuses with strong echo foci in different parts of the fetus.Methods The clinical data were retrospectively analyzed in 82 pregnant women with strong echo foci in different parts of the fetus detected by prenatal ultrasonography.Results In 82 fetuses,1 case of strong echo foci in bilateral ventricles showed ventricular dilatation and left brain parenchymal echo enhancement with clear boundary,3 cases of strong foci in the liver showed spot-like strong echo and indistinct acoustic shadowing,3 cases of abdominal strong echo showed fetal abdominal calcification and fetal bowel dilatation in different degrees in 2 cases,and mixed echo mass in the abdominal cavity,long bone-like structure and solid tissue,cystic cavity and other structures in 1 case;and 75 cases of cardiac hyperechoic foci showed strong echo mass in left ventricle lateral wall in 1 case,multiple calcifications in the heart chordae tendineae in 1,and single calcification of the papillary muscle or the tendon strong echo spot in heart ventricle in 73.Ten cases were lost follow-up（strong echo foci in the heart）.Five cases were terminated pregnancy,including 1 case of strong echo foci in cerebral parenchymal,2 cases of multiple calcification in the liver and 2 cases of strong echo foci in the heart.Three fetuses with strong abdominal echo received operation after birth,and the prognosis was good.In 64 cases,the calcified lesions disappeared during follow-up,including 1 case of single calcification focus in the liver and 63 cases of strong echo foci in the heart.Conclusion The ultrasound images of strong echo foci in different parts of the fetus are different,and the clinical pregnancy outcomes are commonly fine.
