肱骨近端锁定钢板与空心钉内固定治疗肩关节脱位并肱骨大结节撕脱骨折疗效比较 被引量:11

摘要 目的 探讨肱骨近端锁定钢板与空心钉内固定治疗肩关节脱位并肱骨大结节撕脱骨折的临床疗效及手术注意事项。方法 回顾性分析自2013-08—2016-05诊治的30例肩关节脱位并肱骨大结节撕脱骨折,15例采用肱骨近端锁定钢板内固定(钢板组),15例采用空心钉内固定(空心钉组)。结果 除钢板组1例失访外,其余29例均获得随访,随访时间平均10(8~12)个月。术后切口均一期愈合,仅空心钉组1例内固定失败。钢板组术后3、6个月肩关节功能ConstantMurley评分优于空心钉组,差异有统计学意义(P 〈0.05);但2组术后12个月Constant-Murley评分比较差异无统计学意义(P〉0.05)。结论 相对于空心钉内固定,肱骨近端锁定钢板内固定治疗肩关节脱位并肱骨大结节撕脱骨折更能达到牢固的固定以满足早期功能锻炼要求,最大程度改善肩关节功能,提高患者生活质量,特别是合并肩袖损伤的患者。
