彩色多普勒超声对甲状旁腺增生回顾性分析 被引量:2

摘要 目的探讨应用彩色多普勒超声对甲状旁腺增生的诊断及应用价值。方法选择72例慢性肾功能不全甲状旁腺功能亢进患者,其中男性37例,女性35例;年龄28~70岁,平均年龄47.83岁。行超声检查,并对增生的甲状旁腺进行定位及手术切除,与手术及病理结果相结合,对甲状旁腺位置、大小、回声及血流进行描述,回顾性分析甲状旁腺超声声像图特征。结果手术病理为甲状旁腺增生共253个,彩色多普勒超声检出甲状旁腺增生共239个,彩色多普勒超声检出甲状旁腺增生与手术病理符合率为94.47%。甲状旁腺增生腺体血流分级,0级15个,Ⅰ级55个,Ⅱ级30个,Ⅲ级139个。彩色多普勒超声表现,单纯低回声者179个,低回声内有强回声钙化者58个,还有2个为中强回声;动、静脉血流信号均有,动脉血流阻力指数范围0.45~0.76。结论彩色多普勒超声对甲状旁腺增生诊断及术前定位有重要的临床意义。 Objective To investigate diagnostic and application value of color Doppler ultrasonography in parathyroid hyperplasia. Methods A total of 72 cases of chronic renal insufficiency combined hyperparathyroidism were enrolled, which included 37 males and 35 females, aged 28-70 years old with mean age of 47.83 years old. All of them performed ultrasonography, and parathyroid hyperplasia were located and removed by operation. Combined with operation and pathological result,the position, size, echoes and blood flow of parathyroid glands were described, characteristic of ultrasonic imaging were retrospective analyzed. Results Total of 253 hyperparathyroid hyperplasia were diagnosed by pathology, and color Doppler ultrasonography detected 239 parathyroid hyperplasia, the coincidence rate of parathyroid hyperplasia detected by color Doppler ultrasonography and pathology was 94.47 %. The blood flow classification of parathyroid gland hyperplasia showed that 15 of grade 0, 55 of grade Ⅰ, 30 of grade Ⅱ and 139 of grade Ⅲ. The color Doppler ultrasound showed that 179 of simple hypoechoic, 58 of hyperechoic calcifications in hypoechoic and 2 of moderate-strong echoes. The blood flow signals showed venous and artery, and arterial blood flow resistance index was 0.45-0.76. Conclusion It is demonstrated that color Doppler ultrasound has important clinical significance in the diagnosis and preoperative localization of parathyroid hyperplasia.
